    Abstract Influenza like illness (ILI) is the most common cause of absenteeism in the workplace. This study was designed to assess the effect of anti-influenza vaccination on reducing illness and absenteeism among health care workers (HCWs) of two academic hospitals of Mashhad University of Medical Sciences in Iran from September 2015 through June 2016. In this follow up study 166 health care workers in two academic hospitals were observed for 9 months. Influenza vaccine was administered to 83 HCWs on a voluntary basis in September 2015. Respiratory symptoms and absenteeism due to influenza like illness in vaccinated group were evaluated and compared with unvaccinated group every 3 months. A questionnaire was also administered for collecting the socio-demographic and occupational data such as age, sex, job and etc. in the studied groups. The mean of age in the vaccinated and unvaccinated groups was 36.69±8.43 and 37.12±8.3 respectively (P=0.73). There was no significant difference between two studied groups regarding sex, marital status, hospital, shift of work (fixed or rotational), educational degree, smoking and history of atopy. The individuals who were absent from work due to influenza during the first and second trimester were 13 (15.7%) and 7(8.4%) in the case group and 31(37.3%) and 24(28.9%) in the control group respectively (P=0.003, P=0.001). During 9 months of study 23(27.7%) individuals in case group and 51(61.4%) in control group were absent from work. (P=0.001). In this study the vaccinated group showed lower flu-like symptoms and sickness absenteeism compared with unvaccinated group. Therefore we can conclude influenza vaccination is very effective in reducing sickness absenteeism in hospital personnel and annual influenza vaccination is recommended for this job groups.

    Abstract Introduction: Nowadays, fake doctors, known as “Bandis” in our country Iran, represent themselves as traditional healers and use superstitious ideas for the healing of the ill, especially children. Their activities do not comply with any scientific methods. No research has been focused on this issue. The present study aimed to evaluate the education and social levels of the patients and their reasons for referring to these quackeries. Methods: This cross-sectional study was conducted on 1,300 children who were managed in the pediatric clinics of Dr. Sheikh Hospital for children, which is a tertiary care center in Mashhad, Iran. Informed consent was obtained from the guardians and patients. Upon examination, the parents of the children were asked to complete a checklist for data collection. Data analysis was performed in SPSS version 16, and the P-value of less than 0.05 was considered statistically significant. Results: In total, 1,300 children (621 females and 679 male) aged 2.5-12 years (mean age: 5.3±2.9 years) were enrolled in the study. Among the guardians, 62.5% were familiar with traditional healers, and 457 (37.2%) of the respondents had visited these individuals at least once. In addition, 106 of these cases had visited traditional healers more than three times. The parents were enquired about the reasons for visiting traditional healers instead of physicians, and 46.7% believed the applied methods by these individuals to be more effective. Moreover, 3.1% mentioned the lower treatment costs compared to physician visits, 2.9% reported the acute diseases of their children that did not respond to medical treatments, and 47.3% acknowledged those methods as real traditional practices. Conclusion: According to the results traditional healers, are a major public health concern. For unjustified reasons, many people prefer their methods to conventional medicine. Therefore, healthcare authorities must raise public awareness in this regarding through systematic plans in order to prevent future complications.

    Abstract Metabolic syndrome and its various manifestations are considered to be a significant health epidemic in the developed and developing countries across the world. Metabolic syndrome is characterized by a series of metabolic abnormalities, such as central adiposity, insulin resistance, hypertension, glucose intolerance, and dyslipidemia. Patients with metabolic syndrome are at a higher risk of major complications, including fatty liver, type II diabetes mellitus, and cardiovascular diseases. Nuclear receptors are the key regulators of gene transcription, as well as several metabolic pathways. Among these receptors, LXRα and β play a major role in the regulation of lipogenesis, cholesterol/glucose homoeostasis, and inflammatory pathways through the induction or repression of target genes. In addition to metabolic homeostasis and diseases, lipogenesis and hypertriglyceridemia are regarded as the most significant adverse effects of liver X receptor (LXR) activation. Given the importance of lipid and carbohydrate metabolism and inflammation in the development of metabolic disorders, the present study aimed to review the impact of LXR signaling on the risk of metabolic syndrome and its phenotypes, with an emphasis on their potential therapeutic applications in the treatment of metabolic syndrome. In general, growing evidence supports the notion that LXRs may represent the potential drug targets for the treatment of metabolic syndrome.

    Abstract Background: In recent years, the role of Vitamin D (VitD), as an immunomedulator in autoimmune diseases, has been evaluated in basic science and practice. There is a considerable volume of data on the effect of VitD position in lupus and rheumatoid arthritis exacerbation.
    Objective: This study aims to compare VitD serum values in lupus (SLE) and Rheumatoid Arthritis (RA) in the geographical region of northeastern Iran.
    Methods: Lupus and RA Patients were selected with various disease activity levels. All the patients received an equal amount of VitD supplementation and were selected by the same inclusion and exclusion criteria. VitD serum values were measured by a commercial ELISA kit. Data were analyzed in SPSS-15.
    Results: A total of 148 SLE and 156 RA patients were studied. VitD serum levels were 66.54±41.2 nmol/l in the SLE group and 83.74±46.45 nmol/l in the RA group. Statistical analysis showed that VitD serum levels were lower in lupus patients than RA ones (p=0.006).
    Conclusion: VitD serum values were lower in lupus patients than RA ones. Since VitD deficiency is very common in Iran, physiologic doses of VitD supplementation in patients lead to higher serum levels of VitD. Lower VitD values in lupus patients compared with RA ones may stem from intestinal malabsorption, higher doses of corticosteroid therapy, renal involvement and proteinuria, different polymorphisms of VitD receptors, and more sun protection strategies in lupus patients.

    Abstract Background Traumatic brain injury (TBI) is among the leading causes of mortality and long-term disability. Prognosis assessment is a primary factor of clinical decision-making by emergency physicians. This study aimed to investigate the prognostic factors of TBI in the patients admitted to a typical emergency department. Methods This prospective cohort study was conducted involving 100 TBI patients. Baseline characteristics, including age, gender, Glasgow Coma Scale (GCS), and vital signs were recorded. Blood tests and brain CT scans were collected. The patients were followed-up three months after the date of admission. The observed outcomes were categorized as recovery without complication, recovery with complication, or death. Statistical analysis was performed using the simple and multivariate binary logistic regression in the software IBM SPSS version 19. Results The most common brain CT scan findings were subarachnoid hemorrhage (21.0%) and epidural hemorrhage (20.0%). In the follow-up performed three months after the admission, 47 patients (47.0%) had died, 39 (39.0%) were suffering from complications, and 14 patients (14.0%) were recovering without complications. Simple binary logistic regression showed that older age (OR=3.28, 95% C.I.=1.27-8.41), minor/moderate head trauma (OR=13.93, 95% C.I.=1.73-112.11), severe head trauma (OR=54.40, 95% C.I.=5.71-517.56) and presence of deep skull fracture (OR=8.92, 95% C.I.=1.04-75.53) were statistically significant predictors of mortality. Multivariate logistic regression showed that mortality chance was higher in elderly (OR=7.45, 95% C.I.=2.02-27.36), minor/moderate head trauma (OR=26.87, 95% C.I.=2.42-298.25) and severe head trauma (OR=127.97, 95% C.I.=9.11-1796.28). Conclusion This study demonstrated that severe head trauma was the most predicted risk factor of poorer clinical outcomes after TBI.

    Abstract Introduction: To date, several randomized trials have compared calcineurin inhibitors, especially tacrolimus, with cyclosporine in patients with steroid-resistant nephrotic syndrome, proposing conflicting results. Use of immunosuppressive therapy for the treatment of resistant nephrotic syndrome remains a matter of debate, and the evidence on its efficacy and safety is inconclusive.The present study aimed to compare the benefits and limitations of tacrolimus and cyclosporine in the treatment of steroid-resistant nephrotic syndrome. Methods: This systematic review and meta-analysis was conducted via searching for the relevant trials performed until January 2018 in electronic databases such as PubMed, Scopus, ScienceDirect, Cochrane Library, and Web of Science. In total, 285 potentially relevant articles were identified, and four articles were selected for the review. A random effects model was used to analyze data, and the heterogeneity of the articles was assessed using Chi-square-based Cochran’s Q and I 2 statistics, and heterogeneity was considered statistically significant with I 2 >50%. The outcomes were presented as relative risk with 95% confidence interval, and P-value of less than 0.05 was considered statistically significant. In addition, meta-analysis was used for further data analysis. Result: Four eligible randomized controlled trials were evaluated in this systematic review. In terms of partial and complete remission, no significant differences were observed between tacrolimus and cyclosporine. However, lack of response to therapy was significantly lower with tacrolimus (RR=0.289; P=0.02) compared to cyclosporine. No significant differences were observed between the drugs in terms of the infection rate, hypertension, ALT/AST elevation, and gastrointestinal symptoms, while nephrotoxicity (RR=0.395; P=0.004) and hypertrichosis (RR=0.018; P<0.001) were significantly lower with tacrolimus.     Abstract Management of children with acute gastroenteritis is based upon dehydration estimation. There is no clinical or paraclinical tool which exactly estimates the dehydration degree. Recently ultrasonographic parameters as inferior vena cava (IVC) diameter and aorta (AO) have been used in some studies for this purpose. This study aims to evaluate the efficacy of ultrasound in detecting mild and moderate degrees of dehydration in children. The study was performed in the emergency department of Dr. Sheikh’s Children Hospital, Mashhad, Iran. Children with mild to moderate degrees of dehydration according to World health Organization (WHO) clinical scale were enrolled. Their inferior vena cava diameters, aorta and IVC/AO ratio were measured before and after fluid therapy using ultrasound. Ultrasound was performed by two pediatric sonographers. 36 patients (mean age of 16.94±11.02 months) entered the study. 11 patients had mild and 25 moderate dehydration according to WHO clinical scale. All 11 patients with mild dehydration received oral rehydration. 13 patients in the moderate dehydration group received intravenous rehydration because of oral intolerance to fluids and recurrent vomiting. IVC diameter and IVC/AO ratio after fluid therapy in children with both mild and moderate dehydration degrees was significantly greater (P<0.001). However, we did not observe any significant difference in aorta diameter before and after fluid therapy. Using Receiver Operating Characteristic (ROC) curve, the proper cut-off point of IVC/AO ratio to differentiate patients with moderate dehydration from mild dehydration is equal to 0.782 with sensitivity and specificity equal to 88% and 45.45% respectively. Further, the area under the ROC curve for this cut-off is equal to 0.569. In conclusion, ultrasonography cannot differentiate between mild and moderate dehydration degrees, but studies with larger population of patients should be performed.
