    Abstract This Research Topic was focused on provision of novel medical technologies worldwide keeping in mind financial sustainability challenge. An exemplary area certainly are oncology pharmaceuticals where prices have increased 10-fold in recent years leading to concerns on affordability. The objective of this collection of studies was to reveal some of the hidden underlying causes of unequal access to the medicines. Another core issue is the growing proportion of out-of-pocket health spending in many world regions. In line with the joint efforts of the editors and authors we received an exceptionally high response worldwide. This E-Book attracted a total of 37 self-standing research submissions out of which 32 ultimately passed external peer review and got published. Base affiliations of the authors spread across academia, pharmaceutical and medical device industry, governmental authorities and clinical medicine. Their home institutions were situated in fifteen different countries inclusive of Japan, Israel, Russia, USA, Germany, Italy, Netherlands, Austria, Spain, Malta, Serbia, Poland, Bulgaria, Hungary and Malaysia. We frankly believe that authors succeeded to cover important literature gaps referring to these world regions. We solicit global professional audience to put our efforts to the test and read this contribution to the health economics literature

    Abstract Head trauma is a common neurosurgical complication. It is encountered daily at neurosurgical outpatient departments or after-hour emergency outpatient departments. In addition, most cases of patients with trauma transported by ambulance involve those with head trauma. Head trauma is a common and unavoidable neurosurgical injury. In case of a head injury, the neurosurgeon should verify the level of consciousness and perform a proper neurological examination. A head computed tomography should be performed immediately for any abnormalities. However, currently, severe trauma is often complicated by head trauma and various other injuries. When medical examinations and treatments focus only on head trauma, preventable trauma death cannot be avoided. Functional prognosis due to secondary brain injury may be exacerbated. This article presents a standardized procedure for the initial care of patients with multiple trauma and head injuries.

    Abstract Recently, utilization of genetic data has become routine in medicine. It is important to consider the use of genetic information in different situations based on the principles of medical ethics. Furthermore, it is necessary to understand the features of genetic information and to adhere to various guidelines in research and clinical practices. In genomic medicine, which will become the mainstream of medicine using comprehensive genetic information, it will be crucial to fully comprehend the suitable handling of secondary results, and to prioritize benefits to the patients. Moreover, developing a system that incorporates appropriate legislation to ensure nondiscrimination of patients on the basis of their genetic information and to provide a forum for ethical issues that will arise in the future is essential.

    Abstract: We describe a 72-year-old man with Popeye's sign. The patient experienced a sudden shock in his right humerus while mowing grass with his right arm using wide sweeps of a scythe. His right upper arm had an obvious bulging appearance after 3 days, indicating a rupture of the biceps tendon.

    Abstract Key clinical message: Mucinous carcinoma of the skin is clinically characterized by good mobility, slow growth, a macroscopically smooth surface, no easy contact bleeding, and no internal flow on color Doppler; it is thus difficult to distinguish from benign tumors.
    Abstract: A 58-year-old man presented to our clinic complaining of a right cheek induration, growing over the previous 6 months. The tumor surface was smooth, pink, did not bleed easily, and approximately 10 mm in size with good mobility. Ultrasonography revealed a well-circumscribed hypoechoic homogenous tumor with posterior acoustic enhancement; color Doppler displayed no internal flow. Preoperative diagnosis was intradermal type nevus or dermatofibroma. An excisional biopsy was performed under local anesthesia. The biopsy specimen unexpectedly showed tumor cells with an epithelial alveolar configuration floating in a mucin lake. The tumor cells contained enlarged nuclei and formed atypical columnar epithelium, arranged in fused, honeycomb-like glandular ducts, indicating mucinous carcinoma.
