    Abstract Advances in treatment have led to improve survival of patients with cancer, however, cancer therapy-related cardiovascular toxicity(CTR-CVT)from cancer treatment remain a leading cause of treatment-associated morbidity and mortality across cancer survivors. Onco-cardiology has become a cardiology subspecialty oriented to the prevention, diagnosis, treatment and follow up of CTR-CVT in cancer patients throughout all stages of cancer therapy and in the survivorship period. The field is rapidly growing and the Japanese Onco-Cardiology Society(JOCS)was founded in October 2017. Although only a few guidelines in the field of Onco-cardiology have been published, the Japanese Society of Medical Oncology and JOCS organized practical guidelines committee for management of Onco-cardiology in 2019 in recognition of the increasing need for guidelines. The committee began the making of recommendations according to Minds Handbook for Clinical Practice Guidelines Development. The guideline-creation committee proposed 16 clinical questions in consideration of PICO format, which described the population, intervention, control, and outcomes. A literature search was performed using the PubMed (MEDLINE), additionally we did a hand searched. After systematic review analysis, we should shift 11 questions to either background questions or future research questions because of little evidence. The guideline of Onco-cardiology was published on March 2023. In this review, I describe outlines of 5 clinical questions.

    Abstract The Hokushin-Gan-pro, consists of Kanazawa University, Shinshu University, Toyama University, Fukui University, Kanazawa Medical University, Ishikawa Prefectural Nursing University, located in Nagano, Toyama, Ishikawa and Fukui prefectures( Hokushin region), conducts educational program to train professionals for management of cancer patients. We have launched 10 graduate student courses and 11 medical staff courses, and are accepting students who exceed our expectation. We give lectures using e-learning system and hold multidisciplinary seminars that utilize the Web conference system. As an overseas staff training, participants learned cancer genomic medicine and cancer palliative care at medical facilities in 3 countries. The regional cancer database project that utilizes cancer registration data from 22 principal hospitals to clarify the characteristics of cancer patients with disabilities, pleural mesothelioma, pleural adenocarcinoma, and pediatric/ adolescents and young adults(AYA)generation cancer patients in Hokushin region. In addition, public lectures were held to provide cancer information to the citizens in Hokushin region.

    Abstract This review discusses immunomodulatory drug (IMiDs) sequencing and IMiD-free interval strategies for lenalidomide-refractory myeloma. IMiDs and proteasome inhibitors (PIs) improve clinical outcomes in patients with myeloma; however, refractoriness to lenalidomide, a category of IMiD, predicts poor outcomes. Next-generation IMiDs, such as pomalidomide, are effective even for lenalidomide-refractory myeloma. Therefore, an IMiD-sequencing strategy from lenalidomide to pomalidomide would be desirable. PIs are an antimyeloma therapeutic agent with another mode of action that might restore cereblon, a target of IMiDs; therefore, an IMiD-free interval via class switching from lenalidomide to PIs may be a promising alternative for lenalidomide-refractory myeloma. Additionally, the anti-CD38 monoclonal antibody is a key drug for salvage therapy in anti-CD38 monoclonal antibody-naïve patients. In clinical practice, safety profiles and social convenience can play important roles in the choice of combination therapy. In the future, the selection of optimal treatments should be based on the status of the immunological environment and genetic alterations. This review aims to discuss IMiDs sequencing and IMiD-free interval strategies for lenalidomide- refractory myeloma.
