    Abstract Background/Aim: Data on the comparison of diagnostic yields of 22-gauge (22G) and 25-gauge (25G) needles used in endoscopic ultrasound guided fine needle aspiration (EUS-FNA) biopsy usually include solid pancreatic masses. In our study, we compared the diagnostic yield, safety, and performance characteristics of 22G and 25G needles in the EUS-FNA of various solid lesions in or adjacent to the upper gastrointestinal wall and suspicious lymph nodes. Methods: In this retrospective cohort study, we enrolled patients who underwent EUS-FNA using 22G and 25G needles between August 2018 and January 2020. We compared EUS-FNA results with histological findings in operated patients and long-term clinical follow-up results in non-operated patients. Results: Seventy-nine patients (40 patients with 22G needles) were enrolled. There were pancreatic solid masses in 50 (63.3%) patients, subepithelial lesions in 13 (16.5%), suspicious lymph nodes in 12 (15.2%), and various lesions adjacent to the lumen in 4 (5.1%) patients. The diagnostic yield of 22G and 25G needles were 92.5% and 94.9%, respectively, which were similar (P=0.664). EUS-FNA of 2 pancreatic masses required a crossover from a 22G needle to a 25G needle due to lesion stiffness. The technical success rate for the lesion type was 100% and 95% for 25G and 22G needles, respectively (P=0.160). No major complications were observed with either needle. Conclusions: The 25G needle was not superior to the 22G needle in terms of diagnostic yield and safety profile in EUS-FNA of solid lesions. The use of 25G needles in hard masses can provide ease of puncture.

    Abstract Objective:Appendectomy and tonsillectomy remain controversial environmental factors in the etiology of inflammatory bowel disease (IBD). In this study, we aimed to investigate the frequency of tonsillectomy and appendectomy in patients with IBD. In addition, the effects of tonsillectomy and appendectomy on age of IBD diagnosis, site of intestinal involvement, and medical treatments were investigated.Methods:A total of 131 IBD patients and 76 healthy controls (HCs) were included in this study. All of these patients included in this study were asked if they had undergone an appendectomy or tonsillectomy and their age at the time of the surgery. The age of diagnosis of IBD, medical treatment, disease location, and perianal involvement characteristics of the patients collected from the hospital records were evaluated.Results:Of 90 patients with ulcerative colitis (UC), 2 patients underwent appendectomy after the age of 20 and after the diagnosis of UC. Compared with HCs, appendectomy and the age during operation were not associated with the incidence of UC. Moreover, 8 of 41 patients with Crohn’s disease (CD) had undergone appendectomy. Of these, 6 patients had undergone surgery before the age of 20 and before the diagnosis of CD. Compared with HCs, appendectomy and the operative age were significantly associated with the incidence of CD. No significant relationship was observed between the groups in terms of tonsillectomy.Conclusion:Appendectomy was associated with an increased risk of developing CD; however, no effect was observed on the risk of developing UC. Moreover, tonsillectomy was not associated with an increased risk of incidence of both CD and UC.

    Abstract Aim: It has been suggested that early life factors may affect the risk of inflammatory bowel disease by affecting the gut microbiome. Delivery type, breast milk, and parental smoking are the most important environmental factors. There are limited studies on the effects of these factors on the location of the disease and age at diagnosis. In addition, the effects of these factors on medical treatments and bowel involvement are unknown. We examined the relationship between potential early-life risk factors experienced in the first years of life and ulcerative colitis (UC). Material and Method: This study is a prospective case-control study. Sixty-nine UC patients were compared with 44 age- and sex-matched healthy controls (HC). We investigated delivery type, breastfeeding, maternal age at birth, and parents' smoking status, which may be potential early-life risk factors. Our analysis involved the relationship of these potential factors with the age at diagnosis and medical treatments in UC patients. Results: UC and HC groups were compared in terms of delivery type, breastfeeding, and parents' smoking status: There was no statistical difference between the groups. In terms of the duration of breastfeeding, we found that 6-12 months of breastfeeding was lower in the UC group (P=0.046). In addition, the age at diagnosis of the disease was lower in UC patients who were not breastfed and whose mothers smoked (P=0.031, P=0.016, respectively). Conclusions: The duration of breastfeeding is important for the development of UC. We recommend breastfeeding for longer than six months to prevent UC. Maternal smoking cessation has a protective role in the risk of early-onset UC.

    Abstract Objective:The gamma-glutamyl transpeptidase/platelet ratio (GPR) is a newly developed non-invasive serum marker of significant fibrosis and cirrhosis in patients with chronic hepatitis B (CHB) in West Africa. This study aimed to compare the diagnostic accuracy of the GPR with the aspartate aminotransferase-platelet ratio index (APRI) and with the fibrosis-4 (FIB-4) index for detecting liver fibrosis in Turkish patients with CHB.Methods:Seventy-nine patients with CHB who had undergone liver biopsy were included, and GPR, APRI, and FIB-4 data were obtained. The receiver operating characteristic (ROC) curves and area under the ROC curves (AUROCs) were compared.Results:As regards the fibrosis stages of 79 patients, 5 were F0 (6.3%), 17 were F1 (21.5%), 23 were F2 (29.1%), 23 were F3 (29.1%), and 11 were F4 (13.9%). The AUROCs of the GPR and APRI were similar in the diagnosis of significant fibrosis (0.70 vs. 0.69; p=0.928), advanced fibrosis (0.80 vs. 0.72; p=0.174), and cirrhosis (0.73 vs. 0.75; p=0.771) groups. The AUROCs of the GPR and FIB-4 index were similar for diagnosing significant fibrosis (0.70 vs. 0.79; p=0.090) and advanced fibrosis (0.80 vs. 0.77; p=0.569). However, the AUROCs of the FIB-4 index for diagnosing cirrhosis was significantly higher than those for the GPR (0.73 vs. 0.90; p=0.024) and APRI (0.75 vs. 0.90; p=0.024).Conclusion:The GPR can be used to detect significant fibrosis, advanced fibrosis, and cirrhosis, but was not superior to the APRI or FIB-4 index. FIB-4 index performed better than the GPR and APRI for diagnosing cirrhosis.

    Abstract Aim: Pentraxin 3 is a molecule that is known to show inflammation, however few studies with contradictory results have been conducted in current literature in Inflammatory Bowel Disease patients regarding PTX3. We aimed to investigate whether PTX3 can be used in the discrimination of Crohn’s disese and Ulcerative colitis patients and whether there is a correlation of PTX3 with disease activity, involved intestinal segments and their length. Method: Ethical committee approval was obtained for this a prospective, single-centered, single-blind study. IBD patients who were older than 18 years old, without pregnancy, malignancy, acute/chronic infections or inflammatory diseases were included. Healthy volunteers were included in study as a control group. PTX3 levels were measured in the blood. Shapiro-Wilk, Mann-Whitney U, Kruskal Wallis, Bonferroni-Dunn, Spearman tests were used with NCSS 2007 program. Results: The median age of the 56 male and 42 female participants was 37 years (25 CD, 43 UK and 30 controls). In the UK group, PTX3, which did not have relationship with the Trulove Witts score or the Mayo score; was a statistically significant higher in patients with pancolitis. There was no relationship between PTX3 levels and involvement, behavior pattern, CDAI scores and activity of the disease in CD group. Conclusion:PTX3 is not an appropriate biomarker for monitoring the activation of İBD and it can’t distinguish two subtypes from eachother. PTX3 is insufficient to indicate both the behavior pattern and the location of involvement in CD patients. Only, until the control endoscopy times of the patients with UK, it can be monitored whether the disease affects the entire colon by following the PTX3 levels in the interim period.

    Abstract Objective:The Coronavirus Disease-2019 (COVID-19) pandemic continues to be a concern, especially to people with chronic diseases across the entire world. During this pandemic, we undertook an investigation to assess how the disease state and medical treatments of inflammatory bowel disease (IBD) are affecting the IBD patients, their risky perceptions, as well as the frequency and course of COVID-19.Methods:During the pandemic, the information on the course of the disease, medical treatment status of the patients with IBD, and the course of the disease in IBD patients who had COVID-19 were collected via telephonic interview.Results:A total of 102 IBD patients, including 62 with ulcerative colitis and 40 with Crohn’s disease were included in the study. Of these, 52.9% of the patients believed that having IBD was a risk for COVID-19. During the pandemic, 18.6% of the patients did not take their medication regularly for various reasons. Of all, 64.28% of the patients with active disease could not go to the hospital to avoid the risk of acquiring COVID-19 and 4.90% acquired COVID-19.Conclusion:During the pandemic, both the regular consumption of medicines and the reservations regarding admission to hospitals affect the course of IBD. Therefore, the development of strategic action plans to support and manage changes experienced during the course of this disease is expected to facilitate the management of the disease process in a healthier way for both the physicians and the patients.

    Abstract Aim: There is still a need for an ideal laboratory test that can determine the type of disease, the degree of its activity, predict its course, and monitor treatment response in patients with inflammatory bowel disease (IBD). This study aims to investigate the relationship between disease types and activity with Adenosine deaminase (ADA) levels in patients with IBD. Methods: A total of 92 patients with IBD [43 with Crohn’s disease (CD) and 49 with ulcerative colitis (UC)] and 31 healthy control (HC) volunteers were included in this case-control study. Patients’ age, gender, body mass index, location and severity of the disease, medication, endoscopic examination, hemogram, C-reactive Protein (CRP), and ADA results were evaluated.     Abstract Aim: The inlet patch, also called as the heterotopic gastric mucosa, is often located in proximal esophagus and is generally asymptomatic. These lesions are rarely polypoid and mostly have a patchy pattern. In the literature, there are only few cases of inlet patches that show a malignant progression following a metaplasia- dysplasia- adenocarcinoma sequence. In our study, we aimed to present the endoscopic and morphological features of our case series. Methods: The study, which was conducted in two clinics, included the inlet patch cases diagnosed in esophagus biopsy materials between 2016 and 2019. The slides and the demographic data of the cases were re-analyzed. Results: Among 4190 cases whose esophageal biopsies were examined, 63 inlet patches (1.5%) were diagnosed. Thirty four cases were male and 29 were female. Age range was 16-82 years. The cases were mostly located in proximal esophagus and the size of the lesions ranged between 0.2-3.5 cm. One of the four cases with polypoid appearance was microscopically diagnosed as a hyperplastic polyp. The most common gastric type epithelium was the oxyntic type. Helicobacter pylori was observed in four cases, and intestinal metaplasia and low-grade dysplasia were observed in one. No malignancy was diagnosed in our series. Conclusion: Although benign, the inlet patch is significant due to its risk of progression to malignancy. Thus, obtaining biopsies from each lesion endoscopically considered as inlet patch is recommended. This will help determine the precise incidence of the inlet patch and more importantly, identify neoplasia earlier.

    Abstract Aim: The majority of studies on cardiac dysfunction in patients with cirrhosis are associated with left ventricular (LV) myocardial dysfunction, with fewer studies on right ventricular (RV) function. The aims of this study were to evaluate biventricular function, particularly RV function, in patients with cirrhosis and investigate its relationship with disease severity. Methods: We included 50 hospitalized and ambulatory patients with cirrhosis and 33 healthy controls. Echocardiographic evaluation was performed with speckle-tracking analysis. Liver disease was graded according to Child-Pugh and Model for End-Stage Liver Disease (MELD) scores. Results: In our study, the most prevalent etiology of cirrhosis was viral hepatitis (60%), followed by non-alcoholic fatty liver disease (24%).     Abstract Endoscopic procedures are usually carried out in order to diagnose the cause of iron deficiency anemia (IDA). We aimed to evaluate endoscopic findings of our patients with iron deficiency anemia retrospectively.Materials and methods: We retrospectively reviewed our computerized data of patients who attended to Haydarpasa Numune Education and Research Hospital Gastroentology Endoscopy Unit between 2008 and 2010 years. Patients in whom upper gastrointestinal endoscopy and/or colonoscopy were performed in order to diagnose the cause of IDA were included in this study.Results: This study included 490 patients with iron deficiency anemia in whom upper gastrointestinal endoscopy and/or colonoscopy were performed. The mean age of patients was 58.94 ± 14.8 years. Of these patients, 288 (172 female, 116 male) underwent only upper gastrointestinal endoscopy, 116 (61 female, 55 male) underwent only colonoscopy and 84 (39 male, 45 female) underwent both upper gastrointestinal endoscopy and colonoscopy. The rates of failure to reveal the cause of IDA in patients in whom either upper gastrointestinal endoscopy or colonoscopy had been performed were 18.75% and 46.55%, respectively. On other hand, the ratio of failure to reveal the cause of IDA was 3.48% in patients in whom both upper gastrointestinal endoscopy and colonoscopy had been performed.Conclusion: When referring any patient with iron deficiency anemia to endoscopy unit, ordering both upper gastrointestinal endoscopy and colonoscopy may allow us to find the reason more accurately.
