    Abstract Background Healthcare professionals' involvement and reporting of adverse drug reactions are essential for the success of a pharmacovigilance program. The aim of this study was to assess healthcare professionals (medical doctors, pharmacists, nurses, dentists, midwives, and paramedics) current knowledge, attitude, practices, and barriers regarding pharmacovigilance and adverse drug reactions reporting in multicentral healthcare settings. Methods A cross-sectional face-to-face survey was conducted among currently working healthcare professionals in various hospitals in ten districts of Adana province, Türkiye from March to October 2022. A self-administered, pretested questionnaire (Cronbach's alpha = 0.894 for knowledge, attitudes and practices variables) was used for data collection. The questionnaire's final draft included five sections (sociodemographic/general information, knowledge, attitude, practices, and barriers) with 58 questions. The collected data was analyzed in SPSS (version 25) using descriptive statistics, the chi-square test, and logistic regression. Results Of the total 435 distributed questionnaires, 412 completed the entire questionnaire, yielding a 94% response rate. The majority of healthcare professionals (60.4%; n = 249) had never received pharmacovigilance training. Among healthcare professionals 51.9% (n = 214), 71.1% (n = 293) and 92.5% (n = 381) had poor knowledge, positive attitudes and poor practices, respectively. Only 32.5% of healthcare professionals kept the record of an adverse drug reaction and only 13.1% reported adverse drug reactions. The profession (medical doctors, pharmacists, nurses, dentists, midwives, and paramedics) of healthcare professionals and a lack of training were predictors of poor adverse drug reaction reporting (p < 0.05). A statistically significant difference in healthcare professionals and knowledge, attitude and practices scores was also observed (p < 0.05).     Abstract Amaç: Bu çalışma, bir eğitim hastanesinde yatan hastalarda rapor edilen advers ilaç reaksiyonlarının (ADR) oranını ve şeklini değerlendirmek için yapılmıştır. Gereç ve Yöntem: Adana, Balcalı Hastanesi'nde bir yıllık retrospektif kesitsel bir çalışma yapıldı. ADR'lerin nedensellik, şiddet ve ciddiyetini değerlendirmek için WHO-UMC / Naranjo algoritma skalası, Hartwig ve Siegel şiddet skalası ve farmakovijilans temas noktası (PVCP) kriterleri kullanıldı. Bulgular: Toplam 61105 yatan hastadan 20 spontan ADRS vakası PVCP’ye sunulmuş olup insidansı %0.03’ e denk düşmektedir. Bunlardan 18 tanesi ADR bildirim formu dahil edilme kriterlerine göre uygun bulunmuştur. Antibiyotiklerle ilişkili ADR'ler (% 38.8) en sık bildirilirken antineoplastik ajanlar (% 22.2) idi. ADR'lerin çoğu deri ile ilişkiliydi (% 33.3). Nedensellik değerlendirmesinde, bildirilen ADR'lerin neredeyse 'si mümkündür ve % 55.5'i orta ciddiyettedir. Ayrıca, ADR'lerin yaklaşık % 83.3'ünün PVCP kriterlerine göre ciddi olduğu bulunmuştur. Toplam hastalar arasında% 55.5'i reaksiyonlardan iyileşmiştir. ADR'ler doktorlar (% 66.7) ve ardından asistan doktor (% 16.7) tarafından rapor edilmiştir.Sonuç: Bu çalışma hastanede yatan hastalarda ADR'lerin önemli bir problem olduğunu göstermektedir. Şiddetli ve ciddi tipte ADR'lerin izlenmesi ve yönetimi, daha iyi hasta bakımı için değerli olacaktır.

    Abstract Backgraund and Objective: "Adverse Drug Reaction" (ADR) is defined as harmful and unintended effects of drugs in normal usage and dose. Pharmacovigilance (PV) deals with identification, monitoring, prevention or minimizing adverse effects of drugs. The aim of this study was to gauge the level of PV knowledge and awareness of health professionals in Karaman,Turkey. Design, Patients and Sample Size: The survey was concluded amongst 161 physician located in the city of Karaman. Our aim was to measure the level of awareness in the subject matter of general PV knowledge of physicians. Interviews were held face to face. Settigs : SPSS 17,0 package program was used during statistical analyses of data. Main Outcome Measures and Results: 158 of 161 physicians answered questionnaire in this study. Although 49,2 % of the physicians who participated in this study mentioned that they knew the definition of PV, only 86% of this group correctly identified PV. It was determined that 11% of physicians received PV training. 80 % of participants said that they have experienced adverse reactions during drug administration, but only 4% physicians said they filled the "Adverse drug reaction form". 18% Physicians knew that drug adverse reactions should be reported to a center, but only 1 person answered the center's name correctly. Conclusions: In this study showed us the knowledge of ADRs and PV are inadequate among doctors working in Karaman, so the ADR reporting among the physician was insufficient. Rigorous service training programs need to be created in order to eliminate this deficiency and more in-depth information on PV must be provided in medical schools and after graduation.

    Abstract Amaç: Bu çalışma akut apendektomide cerrahi antibiyotik profilaksisinin (SAP) rasyonel kullanımını araştırmak için yapıldı. Gereç ve Yöntem: Bu araştırma, eğitim hastanesinde (Pakistan) altı aylık çalışma süresi boyunca akut apendektomi yapılan 400 hastanın reçetesini prospektif olarak analiz etti. SAP'nin ödenekleri Amerikan Cerrahlar Koleji ve Amerikan Sağlık Sistemi Eczacıları Birliği (ASHP) kurallarına göre değerlendirildi. Antibiyotik kullanımı için Dünya Sağlık Örgütü Anatomik Tedavi Kimyasal (ATC) metodolojisini de kullandı. Bulgular: Bu hastaların yaş ortalaması 28 idi (IQR = 16-45; SD = 8.5). Bunlardan% 37,3'ü doğru seçim, doz için% 100, rota için0 ve antibiyotik zamanlaması için,5 (optimal değer0) ile ilgili kurallara göre yapışmıştır. Sık ve uygunsuz olarak reçete edilen antibiyotik seftriaksondu. SAP'nin kombinasyon kullanımı da rapor edildi. Sonuç: Uygun olmayan seçim, antibiyotiklerin zamanlaması ve yanlış kullanımı bu çalışmada bildirilen başlıca problemlerdir.

    Abstract Purpose: We investigated the effects of Quercetin and Coenzyme Q10 antioxidant, on gentamicin-induced renal failure in rats. Amaç: Gentamisinle böbrek yetmezliği oluşturulan sıçanlarda Quercetin ve CoQ10 nun antioksidan etkilerini araştırmak amaçlanmıştır.Gereç ve Yöntem: Sıçanlara gentamicin (100 mg/kg/day, i.m., günde 1 kez), gentamicin (100 mg/kg/day, i.m.) + Quercetin (15 mg/kg i.p. günde 1 kez) and gentamicin (100 mg/kg/day, i.m.) + CoQ10 (8 mg/kg i.m. günde 1 kez) uygulandı. Bulgular: Kontrol grubundaki BUN değeri 26.8 ± 0.8 (mg/100 mL); buna karşın gentamisin grubunda 117.3 ± 15.4 bulunmuştur. Böbrek histopatolojik inceleme, bu grupta akut tübüler nekrozu doğruladı. Gentamicin + Quercetin ve gentamicin + CoQ10 uygulanan sıçanlarda biyokimyasal ve histolojik parametrelerde kısmi iyileşme gözlemlenmiştir. BUN değerleri gentamisin, gentamisin + Quercetin and gentamisin + CoQ10 gruplarında sırasıyla 117.3 ± 15.4, 44.0 ± 8.3, 47.8 ± 13.8 bulunmuştur. Gentamisin ile aralarında anlamlı farklılık bulunmuştur. Kreatinin değerleri gentamisin, gentamisin + Quercetin ve gentamisin + Koenzim Q10 ile tedavi edilen gruplarda sırasıyla 4.3 ± 0.6, 0.8 ± 0.1, 1.5 ± 0.4 idi.Sonuç: Bu sonuçlar Quercetin ve CoQ10 uygulanmasının gentamisinle nefrotoksisite oluşturulan sıçanlarda koruyucu etkisi olduğunu göstermiştir.

    Abstract Amaç: Cukurova University Tıp Fakültesi Hastanesinde çalışan araştırma görevlisi doktorların akılcı ilaç kullanımı konusunda bilgi ve tutumlarını değerlendirmek amaçlanmıştır. Gereç ve Yöntem: Çalışma tanımlayıcı tipte bir araştırmadır. Cukurova University Tıp Fakültesi Hastanesinde çalışan farklı uzmanlık alanlarından 128 araştırma görevlisi doktora anket çalışması uygulanarak oluşturulmuştur. Ankette akılcı ilaç kullanımı bilgi ve tutumunu değerlendiren 42 soru bulunmaktadır. Bulgular: Ankette doktorların akılcı ilaç kullanımına yönelik bilgi ve tutumları değerlendirilmiştir. Çalışmaya katılan asistan doktorlardan "Hastaya hastalığını ve nedenlerini anlatırım" diyenlerin oranı % 91.4’dür. "Hastaya tedavi uygulama gerekçelerini anlatırım" diyenlerin oranı % 83.6’dir. "Yazdığım ilaçlarla ilgili bilgi veririm" diyenlerin oranı % 84.6, "tedavi bilgilerini hastaya ya da yakınlarına tekrarlatırım" diyenlerin oranı % 46.9’dır. Doktorların .4’i ilaç fiyatlarına dikkat ederek ilaç seçtiğini belirtmiştir. İlaçlarla ilgili güncel bilgi öğrenmede en sık vademekum (.5) kullanılmaktadır. Sonuç: Üçüncü basamak sağlık kuruluşunda yaptığımız çalışmada, doktorların akılcı ilaç kullanımı konusunda bazı uygulamalarda bilgi ve tutumlarının eksik olduğu görülmüştür. Bu durum üçüncü basamakta çalışan araştırma görevlisi doktorlar için akılcı ilaç kullanımı konusunun sürekli tıp eğitimi kapsamında güncellenerek devam etmesi gerekliliğini ortaya koymuştur.

    Abstract Pharmacovigilance, is a scientific study related to follow-up problems in drug administrations and identify, record, announce and take due precautions for these problems. Pharmacovigilance works on to increase safety and effectiveness of drugs and minimize the risks associated with drug use. It is significantly developed and continues to improve to satisfy the recent needs. While information spreads like wildfire in the world, there is a need for strategic planning which adopts an integrated approach to go through difficulties caused by increased transmission between borders, easy acceptance of medical product diversity and increased safety expectation of public.     Abstract Background: A judicious utilization of antibiotic prophylaxis and postoperative analgesics are an effective strategy to prevent surgical site infections and pain, respectively. Aims: The aim of this study was to assess prescription practices and guidelines adherence of antibiotics and analgesics in total knee replacement (TKR) surgery. Setting and Design: A cohort observational pilot study was conducted for a period of 2 months at two tertiary care teaching hospital (government hospital = GH and private hospital = PH) in Islamabad, Pakistan. Subjects and Methods: All the individuals aged ≥18 who underwent TKR surgery with no previous surgery and infection (n = 300) were included during the study period. The patient medication profile was analyzed through various indicators. International evidence-based guidelines were used as a reference to analyze current treatment practices. Statistical Analysis Used: Descriptive statistics such as frequencies, percentages, averages, standard deviation, and Chi-square test was used for interpretation of data in SPSS V-22.0. Results: Preoperative prophylactic antibiotics were prescribed in 94% (n = 283) cases. Of these, 61.5% adhered according to guidelines with respect to correct choice, 64.6% for timing, and 100% for the route (optimal value 100%). The prescribing behavior with respect to antibiotic choice and timing was different among GH and PH hospitals (P = 0.001). Cefazolin was the most commonly prescribed preoperative and postoperative antibiotic, followed by cefuroxime. With respect to analgesics, paracetamol and tramadol were frequently used in TKR patients. One hundred and seventy six (58.6%) patients received analgesics according to the guidlines recommendations, and 7.8% (n = 23) received more than one analgesic in their postoperative prescription. Conclusion: The low compliance rate with guidelines was observed. The choice and timing of antibiotic, selection of analgesics, and its multiple usage were main identified problems.

    Abstract Irrational drug use is one of the main health problems all over the world, especially in developing countries. Limited studies about irrational drug use examples showed that main defects. These are polypharmacy, use of drug drug incompatible with diagnosis, inappropriate use of antibiotics, unnecessary use of expensive drug and people's self-treatment with over the counter and prescribing drugs. There are many reasons for irrational drug use. The main reasons are lack of education, socio-cultural, economic and resulting from regulatory mechanism factors. These reasons affect each other, so problems become more complicated. Reasons resulting from pharmacist and doctors occurs basic of irrational drug use. So attitude of doctors and pharmacist toward rational drug use should be evaluated for resolve defects, formal and non-formal education should be continually used and improved. Rational drug use policies should be developed involving drug companies, doctors, pharmacists and patients. [Archives Medical Review Journal 2015; 24(4.000): 452-462]
