Abstract Various immunity-related adverse events have been reported to be associated with the inhibition of programmed cell death receptor 1. We report a rare case of a relapse of lupus nephritis (LN), involving rapidly progressive glomerulonephritis, which was induced by nivolumab treatment in a patient with oral cancer. The patient had a history of systemic lupus erythematosus and underwent treatment with steroids, rituximab, and plasmapheresis. However, her renal function did not improve, and she died of multiple organ failure. To our knowledge, this is the first description of severe LN induced by nivolumab.

Abstract Treball de Final de Màster Universitari en Gestió Financera i Comptabilitat Avançada (Pla de 2013). Codi: SRL031.

Abstract This study aims to analyze the prognostic significance of angiographic microvascular resistance (AMR) derived from functional coronary angiography in diagnosing coronary microvascular dysfunction (CMD) among patients with chronic coronary syndrome (CCS), particularly focusing on the prognostic significance of CMD in smokers. Additionally, this study assesses the prognostic significance of the diagnostic accuracy of the AMR index for CMD in CCS patients within the smoking population. Seventy-five CCS patients were studied for IMR measurement between June 2018 and December 2020, classified into CMD and non-CMD groups. Pearson correlation analyzed AMR and IMR relationship, while ROC curves determined AMR's predictive value for CMD with optimal cutoff value. An additional 223 CCS patients were included for AMR measurements. The study's primary endpoint was MACE occurrence, followed up through telephone calls. COX one-way analysis and Kaplan-Meier analysis evaluated the association between AMR and MACE risk. In the study, 52% of patients were diagnosed with CMD, and the AUC under the AMR curve for predicted IMR ≥ 25 was 0.90 (95% CI: 0.79-0.95). The CMD group had a higher proportion of female patients, smokers, and those with hypertension and diabetes mellitus compared to the non-CMD group (all P < .05). The CMD group exhibited higher AMR values than the non-CMD group (3.20 ± 0.5 vs 1.95 ± 0.45, P < .01), with a significant correlation between AMR and IMR (R2 = 0.81, P < .001). The ROC curve illustrated AMR's effectiveness in diagnosing CMD in CCS patients, using IMR as the gold standard, with the optimal cutoff value of AMR = 2.6mmHgs/cm (sensitivity: 90.5%, specificity: 81.5%). Patients with AMR ≥ 2.6mmHgs/cm had a higher risk of MACE events (HR = 6.00; 95% CI: 1.59-22.67). AMR in patients with CCS was significantly correlated with IMR and demonstrated good diagnostic accuracy. AMR ≥ 2.6 mm Hg*s/cm was associated with an increased risk of MACE occurrence and served as a valuable prognostic factor. In patients with CCS, AMR has a high diagnostic performance, In patients with CCS who smoke, impaired coronary microvascular function as assessed by AMR is significant and is an independent predictor of MACE.

Abstract Purpose: To evaluate the histologic healing process and mechanical characteristics of the interface between a fascia lata autograft and supraspinatus muscle by establishing a supraspinatus tendon reconstruction model for chronic massive, irreparable rotator cuff tears (MIRCTs).
Methods: A total of 40 rats were studied. Eight rats were sacrificed to establish an intact control group, and the other rats were first established as the chronic MIRCTs model and then developed as the supraspinatus tendon reconstruction model. Histology, fatty infiltration, mechanics and the open field test for the interface between the fascia lata autograft and muscle were assessed at 2, 4, 8 and 16 weeks postoperatively.
Results: Histologically, the interface between the fascia lata autograft and muscle gradually regenerated structural similarities to the normal muscle-tendon interface by 16 weeks postoperatively. The amounts of collagen I and III were both increased significantly during the healing time and stabilized at 8 weeks postoperatively. Fatty infiltration was obvious in the supraspinatus muscle 4 weeks after the MIRCTs model. However, the degree of fatty infiltration in the supraspinatus muscle gradually decreased after supraspinatus tendon reconstruction and stabilized at 8 weeks postoperatively. The ultimate failure force and the ultimate stress gradually increased from 2 to 16 weeks and reached the level of the intact control tendon at 16 weeks postoperatively (P=0.086). The movability of the forepaw returned to normal in the open field test (P=0.907).
Conclusions: In this rat supraspinatus tendon reconstruction model, fascia lata autografts showed good interface healing with the supraspinatus muscle, and fatty infiltration in the supraspinatus muscle was histologically decreased. The interface between the fascia lata autograft and muscle can sustain mechanical strength similar to the anatomical muscle-tendon interface.
Clinical relevance: The supraspinatus tendon reconstruction technique using fascia lata autografts might be a good histological and biomechanical option for treating MIRCTs.

Abstract Developing vehicle finite element (FE) models that match real accident-involved vehicles is challenging. This is related to the intricate variety of geometric features and components. The current study proposes a novel method to efficiently and accurately generate case-specific buck models for car-to-pedestrian simulations. To achieve this, we implemented the vehicle side-view images to detect the horizontal position and roundness of two wheels to rectify distortions and deviations and then extracted the mid-section profiles for comparative calculations against baseline vehicle models to obtain the transformation matrices. Based on the generic buck model which consists of six key components and corresponding matrices, the case-specific buck model was generated semi-automatically based on the transformation metrics. Utilizing this image-based method, a total of 12 vehicle models representing four vehicle categories including family car (FCR), Roadster (RDS), small Sport Utility Vehicle (SUV), and large SUV were generated for car-to-pedestrian collision FE simulations in this study. The pedestrian head trajectories, total contact forces, head injury criterion (HIC), and brain injury criterion (BrIC) were analyzed comparatively. We found that, even within the same vehicle category and initial conditions, the variation in wrap around distance (WAD) spans 84-165 mm, in HIC ranges from 98 to 336, and in BrIC fluctuates between 1.25 and 1.46. These findings highlight the significant influence of vehicle frontal shape and underscore the necessity of using case-specific vehicle models in crash simulations. The proposed method provides a new approach for further vehicle structure optimization aiming at reducing pedestrian head injury and increasing traffic safety.

Abstract Recently, the Heart Failure Society of America, Heart Failure Association of the European Society of Cardiology and Japanese Heart Failure Society jointly issued a consensus statement on Universal Definition and Classification of Heart Failure. Natriuretic peptides were written into the universal definition of heart failure and became the second cardiac marker written into the definition of disease after troponin. The consensus proposed new four classes of heart failure according to ejection fraction(EF), including heart failure with reduced EF(HFrEF), heart failure with mildly reduced EF(HFmrEF), heart failure with preserved EF(HFpEF)and heart failure with improved EF(HFimpEF). Universal Definition and Classification of Heart Failure has wide applicability, which is helpful for clinicians, researchers and patients to understand and adopt. It is an important first step forward to the standardization of diagnosis and treatment of heart failure. While using the universal definition and classification of heart failure, clinical staff should actively conduct scientific research to promote the standardization of diagnosis and treatment of heart failure.

Abstract We develop a unified theory and methodology for the inference of evolutionary Fourier power spectra for a general class of locally stationary and possibly nonlinear processes. In particular, simultaneous confidence regions (SCR) with asymptotically correct coverage rates are constructed for the evolutionary spectral densities on a nearly optimally dense grid of the joint time-frequency domain. A simulation based bootstrap method is proposed to implement the SCR. The SCR enables researchers and practitioners to visually evaluate the magnitude and pattern of the evolutionary power spectra with asymptotically accurate statistical guarantee. The SCR also serves as a unified tool for a wide range of statistical inference problems in time-frequency analysis ranging from tests for white noise, stationarity and time-frequency separability to the validation for non-stationary linear models.

Abstract Considering the sensitivity of the tailless full-wing configuration unmanned aerial vehicle to disturbance and the strong nonlinearity and coupling caused by the manipulation combining the propeller and rudder, a finite-time terminal sliding mode controller with compound compensation is proposed in this paper to ensure stable attitude control. Based on singular perturbation theory, the inner loop sliding mode controller is designed so that the supremum of the convergence time of the rotational angular velocity tracking error in the sliding phase can be directly determined by the control parameters without requiring the initial state, and the outer loop sliding mode controller is designed so that the sliding surface function of the attitude angle tracking error has a fast convergence speed when it is far from and close to the origin. The compound compensation design further addresses the manipulation nonlinearity and disturbance sensitivity of the research object. Based on the minimum Euclidean norm optimization, the designed control allocator achieves command decoupling and reduces the desired control command. Through comprehensive verification, the proposed controller shows superior control performance and low energy consumption and exhibits strong robustness in the cruising flight envelope.

Abstract Breast cancer is the most common malignancy in women worldwide. Triple-negative breast cancer (TNBC), refers breast cancer negative for estrogen receptor, progesterone receptor and human epidermal growth factor receptor 2, characterized by high drug resistance, high metastasis and high recurrence, treatment of which is a difficult problem in the clinical treatment of breast cancer. In order to better treat TNBC clinically, it is a very urgent task to explore the mechanism of TNBC resistance in basic breast cancer research. Pregnane X receptor (PXR) is a nuclear receptor whose main biological function is to participate in the metabolism, transport and clearance of allobiological agents in PXR. PXR plays an important role in drug metabolism and clearance, and PXR is highly expressed in tumor tissues of TNBC patients, which is related to the prognosis of breast cancer patients. This reviews synthesized the important role of PXR in the process of high drug resistance to TNBC chemotherapeutic drugs and related research progress.
