    Abstract Objective: To observe the clinical effect and advantages of dynamic and static acupuncture method combined with manual reduction on posterior semicircular canal benign paroxysmal positional vertigo (PC-BPPV).
    Methods: Ninety patients with PC-BPPV who met the inclusion criteria were randomly divided into manual reduction control group, acupuncture control group and experimental group, with 30 cases in each group. Epley reduction method was used for manual reduction control treatment of patients in the manual reduction control group, until there was no obvious vertigo of patients. Patients in the acupuncture control group received ordinary acupuncture treatment, while patients in the experiment group received dynamic and static acupuncture treatment, both on the basis of manual reduction control treatment.     Abstract Objective: To develop a multimodal deep-learning model for classifying Chinese medicine constitution, i.e., the balanced and unbalanced constitutions, based on inspection of tongue and face images, pulse waves from palpation, and health information from a total of 540 subjects.
    Methods: This study data consisted of tongue and face images, pulse waves obtained by palpation, and health information, including personal information, life habits, medical history, and current symptoms, from 540 subjects (202 males and 338 females). Convolutional neural networks, recurrent neural networks, and fully connected neural networks were used to extract deep features from the data. Feature fusion and decision fusion models were constructed for the multimodal data.
    Results: The optimal models for tongue and face images, pulse waves and health information were ResNet18, Gate Recurrent Unit, and entity embedding, respectively. Feature fusion was superior to decision fusion. The multimodal analysis revealed that multimodal data compensated for the loss of information from a single mode, resulting in improved classification performance.
    Conclusions: Multimodal data fusion can supplement single model information and improve classification performance. Our research underscores the effectiveness of multimodal deep learning technology to identify body constitution for modernizing and improving the intelligent application of Chinese medicine.

    Abstract Studies have found that the substances secreted by the nidamental gland are involved in the formation of the third-level membrane of the fertilized egg and may be closely related to the reproductive activities of cephalopods. However, there is currently a lack in molecular evidence of the involvement of the nidamental gland in reproductive regulation. In this study, RNA sequencing was performed of the nidamental gland tissue of female golden cuttlefish, Sepia esculenta (Hoyle) collected at three key physiological stages: growth stage (NG), spawning stage (NS), and post-reproductive death stage (NA). In total, 65.59Gb of clean data (Q20 > 96%) were obtained. Pairwise comparative analyses of NG vs. NS and NG vs. NA identified 1,889 and 6,574 differentially expressed genes (DEGs), respectively, while only 95 DEGs were identified by comparison of NS vs. NA. Venn diagrams of the three developmental groups identified 7,021 DEGs. By analyzing genes classified as "reproduction" and "reproductive process" in the GO functional classification, the 14-3-3 protein epsilon-like gene and zinc finger RNA-binding protein-like gene were identified. The significantly enriched pathways included the cell cycle, protein digestion and absorption, p53 signaling pathway, mTOR signaling pathway, PI3K-Akt signaling pathway, and lysosomes. Furthermore, the "mucin type O-glycan biosynthesis" and "steroid biosynthesis" pathways related to reproduction were significantly enriched during spawning and senescent stages. This study clarified the regulation of transcript expression in the nidamental gland of S. esculenta at different physiological stages, thereby providing a valuable genomic resource to reveal the role of the nidamental gland in reproduction.

    Abstract Cyclic compounds are widely present in natural products, pharmaceuticals, agrichemicals and materials. The development of novel, high atom/step-economy protocols for the synthesis of various cyclic compounds has attracted much interest in organic synthetic filed. In past two decades, the free radical reactions have been studied extremely, in which the free radical cascade annulations of 1,n-enynes with sulfur/sulfonyl radicals have suggested that this method can be used as a powerful strategy for the construction of various sulfur-containing cyclic compounds. This review summarized the recent advances in the field of radical cascade annulation of 1,n-enynes with sulfur/sulfonyl radicals. Furthermore, we also analyzed the detailed reaction mechanisms and the regioselectivities of 1,n-enynes.

    Abstract Objective: To evaluate outcomes of mixed unicompartmental knee arthroplasty(UKA) and total knee arthroplasty(TKA) in the treatment of medial osteoarthritis(OA) of the knee.
    Methods: Retrospective analysis of 156 patients, 44 males and 112 females, aged from 50 to 75 years old with an average of(58.76±4.97) years old, who underwent knee arthroplasty from October 2017 to October 2019. The patients were divided into two groups:81 cases(81 knees) underwent TKA, including 23 males and 58 females, aged from 51 to 75 years old with an average of (58.60±5.01) years old, and 75 case (75 knees) underwent UKA with mixed phase 3 Oxford, including 21 males and 54 females, aged from 50 to 72 years old with an average of (58.92±4.95) years old. The two groups were compared regarding to the clinical outcomes, assessed using surgical information and complications, American Knee Society score(AKSS) clinical score and functional score. Radiographs were assessed using hip-knee-ankle angle(HKA), tibial component valgus/varus angle(TCVA), tibial component posterior slope angle(TCPSA), femoral component valgus/varus angle(FCVA), femoral component posterior slope angle(FCPSA), looking for bearing dislocation, prosthesis loosening, progression of OA in lateral compartment.
    Results: Intraoperative bleeding, operative time and hospital days were significantly better in the UKA group than in the TKA group (
    Conclusion: Mixed phase 3 Oxford UKA in medial unicompartmental knee osteoarthritis was considerably better than TKA for less blood loss, shorter operation time, shorter hospital stay, rapid postoperative recovery, helping achieve satisfactory function, provided satisfactory outcome.

    Abstract Background: Triamcinolone acetonide (TAC) is widely used for hypertrophic scars and keloids; however, TAC has variable efficacy and safety in different individuals.
    Purpose: To evaluate the efficacy and safety of intralesional TAC for treatment of hypertrophic scars and keloids.
    Data sources: Searches of PubMed, EMBASE, the Cochrane Library, and ClinicalTrials.gov prior to 25 March 2020.
    Study selection: Randomized controlled trials in English that compared TAC with a placebo or other medications that are commonly used for intralesional injection in hypertrophic scars and keloids.
    Data extraction: Primary outcomes were reduction in scar height, vascularity, pliability, pigmentation, total scores on the Vancouver Scar Scale (VSS) or patient and observer scar assessment scale (POSAS), telangiectasia, and skin atrophy. Secondary outcomes included overall scar improvement.
    Data synthesis: Fifteen trials met the inclusion criteria. In the short term, TAC was associated with a significant improvement in vascularity (MD: -0.22, 95% CI: -0.42 to -0.02) and pliability (MD: -0.25, 95% CI: -0.44 to -0.06) compared to verapamil. In the medium term, compared to TAC, 5-FU showed a significant improvement in scar height (SMD: 0.95, 95% CI: 0.15-1.75), while TAC led to a significant improvement in vascularity compared to 5-FU (MD: -0.45, 95% CI: -0.76 to -0.14). Compared to TAC, TAC+5-FU showed a significant improvement in pliability (SMD: 0.98, 95% CI: 0.17-1.78) and pigmentation (MD: 0.45, 95% CI: 0.12-0.78). Botulinum toxin type A resulted in significantly better pliability (SMD: 1.99, 95% CI: 0.98-3.00) compared to TAC. In the long term, compared to TAC, 5-FU led to a significant improvement in scar height (MD: 0.55, 95% CI: 0.17-0.93), but significantly less vascularity (MD: -0.35, 95% CI: -0.65 to -0.05). Compared to TAC, TAC+5-FU produced a significant improvement in scar height (MD: 1.50, 95% CI: 1.12-1.88), pliability (MD: 0.45, 95% CI: 0.10-0.80), and pigmentation (MD: 0.55, 95% CI: 0.24-0.86).
    Conclusion: TAC may be beneficial for the short-term treatment of hypertrophic scars and keloids; however, 5-FU, 5-FU+TAC, and verapamil may produce superior results for medium- and long-term treatments. TAC injections at concentrations of 20 mg/ml or 40 mg/ml are more likely to result in skin atrophy compared to 5-FU or verapamil, and are more likely to cause telangiectasia than 5-FU, 5-FU+TAC, or bleomycin.

    Abstract Objective: To observe the serum levels of Fibrinogen(Fg), Interleukin - 33(IL-33) in patients with cerebral infarction and periodontitis, and to explore the relationship of cerebral infarction and periodontitis. Methods: 104 subjects were included in this study. Among them, 23 were patients with CI and CP (group CI + CP), 28 with CI (group CI) , 28 with CP(group CP) and 25 were healthy volunteers (group H). The periodontal indexes and the serum levels of Fg and IL-33 were measured. Results: In groups of CI + CP, CP and CI the Fg and IL-33 were significantly higher than those in the group H (P<0.01). In groups of CI + CP the Fg and Il-33 was higher than group CI and CP (P<0.05). Conclusion: In patients with cerebral infarction with periodontitis, the levels of fibrinogen and interleukin-33 were both increased, and their concentrations were in direct proportion to the periodontal indexes.

    Abstract Background: Paragangliomas and extra-adrenal pheochromocytomas are uncommon neuroendocrine tumors with ubiquitous distribution. Malignant paraganglioma is a relatively rare entity. We report the treatment and pathological characteristics of a patient with malignant paraganglioma, and summarize the latest advances in the treatment of malignant paraganglioma based on a literature review.
    Case summary: A 45-year-old Chinese woman presented to the hospital due to pain in the waist (right side) and right buttock, and was diagnosed as malignant paraganglioma after the placement of ureteral stent, implantation of ileus catheter, and transvaginal removal of the vaginal mass. After relief of intestinal obstruction, the patient received intravenous chemotherapy and peritoneal perfusion chemotherapy. Although her pelvic mass disease was stable, she developed multiple liver metastases and bone metastases. Due to the development of spinal cord compression, she underwent orthopedic surgery, followed by radiotherapy, and molecular targeted therapy with apatinib, but with poor disease control.
    Conclusion: Clinical management of paraganglioma is challenging for endocrinologists and oncologists. Prospective studies are required to develop standardized therapeutic strategies for malignant paragangliomas.
