    Abstract Background: Final-year undergraduate medical students often do not feel well prepared for their start of residency training. Self-assessment of competences is important so that medical trainees can take responsibility for their learning. In this study, we investigated how final-year medical students self-assessed their competences as they neared their transition to postgraduate training. The aim was to identify areas for improvement in undergraduate training.
    Methods: In the academic year 2019/2020, a national online survey was sent to final-year undergraduate medical students via their respective medical schools. The survey included ten facets of competence (FOC) most relevant for beginning residents. The participants were asked to self-assess their competence for each FOC on a 5-point Likert scale (1: strongly disagree to 5: strongly agree). We established an order of self-assessed FOC performance by means and calculated paired t-tests. Gender differences were assessed with independent t-tests.
    Results: A total of 1083 students from 35 medical schools completed the questionnaire. Mean age was 27.2 ± 3.1 years and 65.8% were female. Students rated their performance highest in the FOCs 'Teamwork and collegiality' and 'Empathy and openness' (97.1 and 95.0% 'strongly agree' or 'agree', respectively) and lowest in 'Verbal communication with colleagues and supervisors' and 'Scientifically and empirically grounded method of working' (22.8 and 40.2% 'strongly disagree', 'disagree', or 'neither agree nor disagree', respectively). Women rated their performance of 'Teamwork and collegiality', 'Empathy and openness', and 'Knowing and maintaining own personal bounds and possibilities' significantly higher than men did (Cohen's d > .2), while men showed higher self-assessed performance in 'Scientifically and empirically grounded method of working' than women (Cohen's d = .38). The FOCs 'Responsibility', 'Knowing and maintaining own personal bounds and possibilities', 'Structure, work planning, and priorities', 'Coping with mistakes', and 'Scientifically and empirically grounded method of working' revealed lower self-assessed performance than the order of FOC relevance established by physicians for beginning residents.
    Conclusions: The differences between the level of students' self-assessed FOC performance and physicians' ranking of FOC relevance revealed areas for improvement in undergraduate medical education related to health system sciences. Final-year students might benefit from additional or better training in management skills, professionalism, and evidence-based medicine. Surveys of self-assessed competences may be useful to monitor competence development during undergraduate training.

    Abstract In Germany there is a threat of a shortage of junior general practitioners (GPs). One third of currently employed GPs are 60 years and older. Every year, 1700 GPs leave the profession, while only about 1350 new GP-specialist approvals are issued. The Advisory Council on the Assessment of Developments in the Health Care Sector estimates that there will be around 20,000 unmet replacement needs by 2025. By 2017, 2600 GP seats were already vacant.Based on the results of the KarMed study, this paper examines the questions as to how many residents intend to become general practitioners after graduation and how they develop during postgraduate training. Furthermore, the consequences of the increasing proportion of female physicians in primary care in terms of the possible volume of work will be investigated. Finally, measures that could reduce a possible personnel shortage are discussed.The attractiveness of general practice has increased considerably during residency over the last six years of observation. The preference for GP work is linked to parenthood. More female doctors strive for part-time work and an employment contract instead of a private practice. On this basis, it can be estimated that the volume of work performed by these female GPs will be half that of the traditional (male) working model in private practice.The forecasted numbers of new recruits are unlikely to be sufficient to cover the demand for GP care in Germany. Possible effective measures would be, for example, the introduction of a gatekeeper system, creation of multi-professional centers, a quota system for access to postgraduate training of specialists, a reformed national planning of GP supply in the country, and a shortening and flexibilization of postgraduate training for general practice.

    Abstract Background: Professional qualification as a doctor of medicine includes completion of medical studies and residency. Data on the real duration of residency are currently not assessed systematically in Germany.
    Objectives: Our study aimed to analyze data on the real length of residency under consideration of area of expertise (specialization), part-time working, grades after school and medical studies, gender, and parenthood of the physicians in residency.
    Material and methods: The KarMed Study's database consists of annual postal surveys throughout the entire residency of medical students, beginning with their "Practical Year" in 2008/2009 until 2019. The study analyzed data six and ten years within this residency period.
    Results: The majority of the residents was capable of finishing their residency within the minimal time. One significant effect on actual duration of residency was the subject choice. One-third of the female residents had not yet finished their residency after ten years. Partenhood had a significant effect on female residents, but not on male residents. A regression analysis showed (R²corr=0.03, p<0.001) that the duration of residency was associated with the grade received after studying (β=0.30). Male residents were more satisfied with the overall residency then female residents.
    Discussion: Structural improvement in postgraduate training needs to be adjusted in terms of parental status. Prospective studies should take the selection process of universities into account and examine its association with the time required to complete residency.

    Abstract The correct and early diagnosis of a rare disease in children is of particular importance in regard to the frequently fateful consequences for young families. Even well-known rare diseases are diagnosed with unacceptable delay in many countries. After decades of studying medical histories with delayed diagnoses and the respective literature, we describe the often severe aftereffects of a late diagnosis. We point out the underlying problems on the part of the physicians involved, of the affected families and of the health system in Germany, in particular with respect to the many emerging centers for rare diseases, most of which are associated with academic institutions. We gained the impression that the cause of delayed diagnoses is frequently not a lack of expertise or other resources, but inefficient communication between parents, practitioners and expert centers. We consider two approaches as promising and practicable: (1) strengthening the parents' competence and role in the dialogue with their doctors, an important element of which is obtaining written information on the state of the diagnostic process in language understandable to the parents; (2) definition of binding requirements for centers officially dedicated to the diagnosis of rare diseases and to research into unknown conditions. Many of our observations and deliberations in the pediatric field should be equally applicable to adults.

    Abstract Hintergrund: Für die ärztliche Qualifikation ist zwischen dem Studium und der sich daran anschließenden fachärztlichen Weiterbildung zu unterscheiden Die reale Dauer des Zeitraums zwischen Approbation und fachärztlicher Anerkennung wird in Deutschland derzeit nicht systematisch erfasst.
    Ziel der Arbeit: Mit dieser Studie soll die reale Dauer der fachärztlichen Weiterbildung in Deutschland untersucht werden. Die vorliegende Arbeit befasst sich mit dem Einfluss der Fachgebietswahl, einer Teilzeittätigkeit sowie der Abitur- und Abschlussnote im Medizinstudium, dem Geschlecht und dem Elternstatus auf die tatsächliche Dauer der fachärztlichen Weiterbildung.
    Material und Methoden: Datenbasis der KarMed-Studie ist die postalischen Befragung einer Kohorte, die im Jahrgang 2008/09 ihr Praktisches Jahr absolviert hat und anschließend während sieben Messzeitpunkten – zuletzt in 2019 – während ihrer fachärztlicher Weiterbildung begleitet wurde. Ausgewertet wurden Daten, die zum sechsten bzw. siebten Messzeitpunkt, sechs und zehn Jahre nach der Approbation erfasst wurden.
    Ergebnisse: Die Mehrheit der Ärztinnen und Ärzte konnte die fachärztliche Weiterbildung in der Minimalzeit abschließen, dabei hatte die Fachgebietswahl einen signifikanten Einfluss auf die Abschlusshäufigkeiten. Bezüglich der Dauer der fachärztlichen Weiterbildung konnte gezeigt werden, dass insbesondere Ärztinnen nach zehn Jahren zu einem Drittel noch keine Facharztanerkennung erreicht hatten. Dabei hatte der Elternstatus einen signifikanten Effekt in Bezug auf den Abschluss der fachärztlichen Weiterbildung bei Ärztinnen, nicht aber bei Ärzten. Regressionsanalytisch (R²corr=0,03, p<.001) ließ sich zeigen, dass der Stand der Weiterbildung mit der Abschlussnote des Medizinstudiums assoziiert ist (β=.30). Die Zufriedenheit mit der fachärztlichen Weiterbildung wurde von Ärzten höher eingeschätzt wird als von Ärztinnen.
    Schlussfolgerung: Die Auswertungen der KarMed Studie zeigen, dass weiterhin Nachbesserungsbedarf besteht, damit die Vereinbarkeit von Beruf und Familie für Ärztinnen gewährleistet werden kann. Zukünftige Studien sollten auch die Untersuchung der Auswahlverfahren der Hochschulen und deren Zusammenhang auf die Dauer der Weiterbildung untersuchen
