    Abstract Hintergrund: Der Übergang zum Erwachsenenalter stellt eine besonders vulnerable Episode in der individuellen Entwicklung des Menschen dar. Bei Menschen mit Epilepsie können krankheitsassoziierte Faktoren die Entwicklungschancen maßgeblich stören, weshalb bei der Weiterbehandlung junger Erwachsener mit Epilepsie nicht nur medizinische Aspekte berücksichtigt werden dürfen.
    Fragestellung: Systematische Darstellung relevanter Aspekte in Transition und Transfer bei Epilepsien sowie gezielte Beleuchtung von Faktoren, die in der Weiterbehandlung von jungen Erwachsenen mit Epilepsie relevant sind.
    Material und Methoden: Zusammenfassung und Diskussion von Original- und Übersichtsarbeiten sowie von Expertenempfehlungen.
    Ergebnisse: Aufgrund der hohen Inzidenz von Epilepsien in den ersten beiden Lebensjahrzehnten, kommt dem strukturierten Übergang in der klinischen Praxis eine entscheidende Bedeutung zu. Krankheitsassoziierte Faktoren führen bei dieser Patientengruppe zu Besonderheiten, die langfristige Auswirkungen auf die individuelle Entwicklung haben und in therapeutische Konzepte einbezogen werden sollten. Ein strukturierter Übergang in die Erwachsenenmedizin sowie die Berücksichtigung verschiedener Schlüsselfaktoren, wie z. B. der Verlauf spezifischer Epilepsiesyndrome, die Wirkung von Antiepileptika sowie soziale Aspekte und individuelle Lebenskonzepte, sind wichtige Aspekte in der Behandlung junger Erwachsener mit Epilepsie.
    Diskussion: Bei jungen Erwachsenen mit Epilepsie sollten verschiedene Aspekte, die über die medizinische Behandlung hinausgehen, berücksichtigt und in die individuelle Behandlungsstrategie einbezogen werden.

    Abstract Background: In rural areas, epilepsy patients have limited access to specialist secondary care. Substantial travel and waiting times of several hours are common. Communication between general practitioners (GP) and specialist epileptologists regarding diagnosis and treatment is further complicated by the high workload on both sides and the different prioritisation of treatment goals. This study aims to investigate the feasibility of an interprofessional audiovisual patient-doctor teleconsultation, and its acceptance in clinical practice in patients with epilepsy in a rural region in Germany.
    Method: Ten patients participated in telemedicine consultations in their GP practice. The practice was located in a sparsely populated region of Mecklenburg-Western Pomerania, and was equipped with technical equipment specifically procured for the project. An explorative qualitative interview was conducted with all participants. We based this paper on the consolidated criteria for reporting qualitative research (COREQ).
    Results: Despite initial uncertainties on the patients' side regarding the consultation setting, all participants found the teleconsultation helpful. Some patients were initially intimidated and felt slightly overwhelmed by the attention provided and the technology used (multiple HD cameras, large high-resolution screens). However, during the consultation, they felt supported by their GP and were satisfied that their needs were addressed in an appropriate and timely manner. The hardware used was not felt to be a nuisance or to interfere with the conversation between doctor and patient. Patients also appreciated the time saved and the organisational convenience compared to a visit to a university outpatient clinic. Most consultations led to therapeutic consequences. Some patients seemed to benefit particularly, for example those who needed a medication change.
    Conclusion: This pilot study provides first evidence that teleconsultations between patients, specialists, and GPs are possible in rural areas. Interprofessional collaboration between GPs and epileptologists can improve the care of patients with epilepsy. Further research should investigate the effectiveness and efficiency of interprofessional telemedicine consultations for epilepsy and other conditions.

    Abstract Background: A significant and growing portion of the global burden of diseases is caused by neurological disorders. Tele-neurology has the potential to improve access to health care services and the quality of care, particularly in rural and underserved areas. The economic evaluation of the stepped wedge randomised controlled trial NeTKoH aims to ascertain the cost-effectiveness and cost-utility regarding the effects of a tele-neurologic intervention in primary care in a rural area in Germany.
    Methods: This protocol outlines the methods used when conducting the trial-based economic evaluation of NeTKoH. The outcomes used in our economic analysis are all prespecified endpoints of the NeTKoH trial. Outcomes considered for the cost-utility and cost-effectiveness analyses will be quality-adjusted life years (QALYs) derived from the EQ-5D-5L, proportion of neurologic problems being solved at the GP's office (primary outcome), hospital length-of-stay and number of hospital stays. Costs will be prospectively collected during the trial by the participating statutory health insurances, and will be analysed from a statutory health insurance perspective within the German health care system. This economic evaluation will be reported complying with the Consolidated Health Economic Evaluation Reporting Standards (CHEERS) checklist.
    Discussion: This within-trial economic evaluation relaying the costs and outcomes of an interdisciplinary tele-consulting intervention will provide high-quality evidence for cost-effectiveness and policy implications of a tele-neurological programme, including the potential for application in other rural areas in Germany or other jurisdictions with a comparable health system.

    Abstract The primary aim of this study was to identify predictors and resilience factors for unemployment and early retirement in patients with epilepsy of working age based on data from a multicenter German cohort study performed in 2020 (n = 456) by using multivariate binary logistic regression analysis. A second aim was to assess the assumed working ability of patients as well as the use of occupational reintegration measures. The unemployment rate was 8.3%, and 18% of patients had retired early due to epilepsy. Multivariate binary logistic regression analysis identified the presence of a relevant disability and frequent seizures as significant predictors of unemployment and early retirement, while seizures in remission were the only resilience factor associated with job retention. Regarding occupational incapacity, at the time of the survey, most of the patients in early retirement or unemployment were fit for work in their original or extended occupational setting. The proportion of patients with recent epilepsy-related occupational retraining (0.4%) or job changes (0.9%) was low, and only 2.4% reported an epilepsy-related reduction in work time. These findings underline the persistent disadvantage of patients with epilepsy in the professional field and the urgent need for effective, comprehensive work reintegration measures that must be made accessible for all patients.
