Abstract	Background: Mature cystic teratomas of the ovary (MCTO) are a type of germ cell tumor that may contain well-differentiated tissues developed from three germ cell layers and constitute about 20% of ovarian germ cell tumors. They are rare ovarian tumors with an annual incidence variable from 1.2 to 14.2 cases per 100,000 that occur mainly in women of reproductive age. They are frequently benign with a slow growth rate, even if they can undergo a malignant transformation in about 1-2% of cases. Case presentation: Here, we present the case of an elderly woman referred to gynecological first aid for acute abdominal pain and showing a giant and unusual MCTO at rapid growth with malignant transformation in squamous cell carcinoma (FIGO stage Ia). The patient underwent pelvic mass removal trough emergency longitudinal midline laparotomic incision with intraoperative frozen pathologic examination. A complete surgical staging during the first surgery was performed. After about 9 years of follow-up, she died of non-oncological reasons without recurrence. Conclusions: Present case highlights that CMTO with malignant transformation should always be suspected in elderly women in presence of pelvic mass at rapid growth, even if in absence of other clinical and ultrasonographic signs of malignancy. An intraoperative frozen pathologic examination may drive the best treatment.

Abstract	Pain is an underestimated finding in myotonic dystrophy type 1 (DM1). We provide a characterization of pain in terms of functional implications through a multidimensional assessment in patients with DM1, focusing on gender differences. We assessed pain through the Brief Pain Inventory (BPI) and its indexes (the Severity Index (SI) and the Interference Index (II)), balance/gait (the Tinetti Performance-Oriented Mobility Assessment (POMA)), functional abilities (the Functional Independence Measure (FIM)), and fatigue (the Fatigue Severity Scale (FSS)). We divided our sample into a mild (<4) and a moderate-severe group (≥4) based on BPI indexes. A between-group analysis was performed. We recruited 23 males and 22 females with DM1. A statistically significant difference was found for the FSS and the BPI-SI ≥ 4, and for all outcomes in the BPI-II ≥ 4 (

Abstract	Background: Acute cholecystitis is an acute inflammation of the gallbladder. It represents one-third of all surgical emergency hospital admissions and has significant socioeconomic impact. Laparoscopic cholecystectomy, regardless of age, is the gold standard for this disease, but the optimal timing of surgical intervention is an open issue since the 2007 Tokyo guidelines. Methods: We recruited from March 2015 to June 2018, in a retrospective study, 144 patients with acute cholecystitis and treated with laparoscopic cholecystectomy. The patients were divided into two groups: group A (N.=66), operated within 72 hours and group B (N.=78), between 72 hours and 1 week after the onset of symptoms. After, the two groups were further stratified by the grade of severity of acute cholecystitis in according to the Tokyo guidelines: in group A, 39 patients were grade I and 27 grade II; in group B, 48 patients were grade I and 30 grade II. Results: The operative time was longer in group B patients versus group A. In group B, there was a greater difficulty in dissecting and detecting the Calot's triangle, more conversions to open, a greater mean length of hospital stay and more post-operative days. In patients with grade II, especially in the group B, were greater inflammation stage, conversions to open, difficulty in the dissection of the Calot's triangle, mean length of hospital stay and post-operative days. The operative timing within 72 hours in patients with grade I, have only advantage in the mean length of hospital stay, while in grade II, the advantages are also in the lesser difficulty in dissecting the Calot's triangle, fewer conversions and fewer post-operative days. Conclusions: Early laparoscopic cholecystectomy for acute cholecystitis should be performed considering not only the onset of symptoms, but above all the grade of severity of AC in according with TG. Grade II, particularly, must be treated within 72 hours and by experienced surgeon.

Abstract	Pain is an underestimated finding in myotonic dystrophy type 1 (DM1). A statistically significant difference was found for the FSS and the BPI-SI ≥ 4, and for all outcomes in the BPI-II ≥ 4 ( p ≤ 0.003). In the female group, all outcomes except for the FIM were statistically significantly worse ( p ≤ 0.004). Dividing our sample into four groups based on gender and the BPI, a statistically significant difference was found for FSS between the two groups with BPI-II ≥ 4 (with worsen score in the female one) ( p < 0.002). Pain in DM1 patients is highly reported and gender related, with increased fatigue and poor balance/gait in the female group.

Abstract	The increasing share of renewable energy sources in energy systems will lead to unpredictable moments of surplus/deficit in energy production. To address this issue, users with heat pumps can provide support to power grid operators through flexible unit operation achieved via Demand Response programs. For buildings connected to low-temperature heating networks with ensured third-party access, further room for flexibility can be explored by investigating the production of surplus heat that can be sold to the network. A key aspect lies in the identification of the energy pricing options that could encourage such flexible operation of a heat pump by “thermal prosumers”. To this aim, the present study investigates the impact of ad hoc variations in the electricity purchasing price through discounts or penalties included in the “network cost” component of the price on cost-effective operation of a heat pump connected to the thermal network. To discuss the effects of different pricing options in terms of increased flexibility, an office building located in Italy and equipped with a high-temperature heat pump is adopted as the case study. A heuristic profit-oriented management strategy of the heat pump is assumed, and dynamic simulations are performed. The results indicate that at current electricity prices, the heat pump operation is profitable both when supplying the heat to meet the building’s requirements and when producing surplus heat for sale to the thermal network. In addition, it is revealed that the penalties applied to the electricity purchasing price are effective in encouraging changes in the heat pump operation strategy, reducing its average production (the building increasingly relying on buying heat from the network) and the associated electricity consumption by 46.0% and 79.7% in the “light” and “severe” local power deficit scenarios, respectively.

Abstract	The efficient utilization of renewable energy sources should rely on the exploitation of a mix of thermal and electric energy rather than relying on a single energy source. One way to apply this shared generation concept to space heating/cooling and refrigeration in both residential and industrial sector is through hybrid sorption-compression chillers. However, the experience on these systems is still limited and therefore their design and optimization require some efforts. Starting from the experimental experience on the testing of different hybrid cascade chillers, and integrating the measurement with a dynamic model, some considerations on the sizing, design and optimization of hybrid thermal-electric chillers are reported. In particular, design conditions of pre-commercial or commercial systems are evaluated and optimization at different levels is proposed, i.e. on the core components (through the proper design of relative capacities of the units in the cascade and through proper selection of the refrigerant), on the auxiliaries, to reduce their electricity consumption, and on the overall management of the hybrid chiller. Results demonstrated that the higher is the operating temperature lift between evaporator and condenser the higher are the achievable energy savings of a cascade chiller.

Abstract	Introduction: Leiomyosarcoma is a rare malignant mesenchymal cancer of smooth muscle and about 50-60 % of cases are from inferior vena cava. Recently, preoperative reconstruction of three-dimensional (3D) images from computed tomography (CT) and intraoperative indocyanine green (ICG) are being developed. We present a case of laparoscopic left nephro-adrenalectomy using ICG fluorescence associated with 3D-CT reconstruction:n. Case report: A 72-years-old female patient presented pain in left side from two months. Contrast computed tomography and magnetic resonance imaging showed a solid mass between left-adrenal gland and upper renal pole. After multidisciplinary evaluation, a laparoscopic left nephno-adrenalectomy was performed. Discussion: The surgery was without intra and postoperative complications. The patient was discharged in the 3rd postoperative day without complications and with oral steroid replacement therapy. Histology confirmed leiomysarcoma of renal vein. Conclusion: In this case, use intraoperative ICG fluorescence associated with preoperative 3D-CT was a valuable support for surgery.
