    Abstract This study investigates the integration and impact of Large Language Models (LLMs), like ChatGPT, in India's healthcare sector. Our research employs a dual approach, engaging both general users and medical professionals through surveys and interviews respectively. Our findings reveal that healthcare professionals value ChatGPT in medical education and preliminary clinical settings, but exercise caution due to concerns about reliability, privacy, and the need for cross-verification with medical references. General users show a preference for AI interactions in healthcare, but concerns regarding accuracy and trust persist. The study underscores the need for these technologies to complement, not replace, human medical expertise, highlighting the importance of developing LLMs in collaboration with healthcare providers. This paper enhances the understanding of LLMs in healthcare, detailing current usage, user trust, and improvement areas. Our insights inform future research and development, underscoring the need for ethically compliant, user-focused LLM advancements that address healthcare-specific challenges.

    Abstract Purpose: Scleral-fixation of intraocular lenses (IOLs) provides an option for eyes that lack sufficient capsular support for in-the-bag IOL placement. The latest techniques for lens fixation include use of a novel suture material, Gore-Tex, and a sutureless method, with flanged intrascleral fixation. The purpose of this pilot study was to compare these methods in terms of anatomic and clinical outcomes.
    Methods: A total of 35 eyes of patients 18-60 years of age who presented with aphakia, subluxated lens, or ectopia lentis were randomized into two groups. Group A (15 eyes) underwent flanged intrascleral IOL fixation using the Yamane technique; group B (20 eyes) underwent 4-point transscleral fixation of IOL using Gore-Tex suture. The following parameters were compared between groups on day 1, week 3, and month 6 postoperatively: logMAR uncorrected and best-corrected visual acuity, retinoscopy, IOL centration on slit-lamp biomicroscopy, and IOL tilt on ultrasound biomicroscopy.
    Results: Postoperative visual acuity was better in group B: uncorrected, logMAR 0.89 ± 0.22 versus 0.72 ± 0.24 (
    Conclusions: In our small study cohort, both sutureless flanged IOL fixation and Gore-Tex sutured scleral IOL fixation resulted in excellent visual rehabilitation of patients with aphakia and subluxated lenses. Patients who underwent Gore-Tex suture fixation experienced better postoperative visual acuity, IOL centration, and stability.

    Abstract Background: Obstructive sleep apnea (OSA) and hypertension are common complications in children with chronic kidney disease (CKD). Progression of CKD can aggravate OSA and hypertension whereas worsening sleep apnea can make hypertension difficult to treat in CKD patients. We, therefore, conducted a prospective study to evaluate the association between OSA and hypertension in pediatric patients with CKD.
    Method: In this prospective observational study consecutive children with CKD stage 3-5 (nondialysis dependent) underwent overnight polysomnography and 24-h ambulatory blood pressure monitoring (ABPM). The detailed clinical features and investigations were recorded in a prestructured performa.
    Results: Twenty-two children completed overnight polysomnography and 24-h ABPM was performed within 48 h of performing polysomnography. The median (IQR) age of the study population was 11 (8.5-15.5) years, with an age range of 5-18 years. Moderate-severe OSA defined as apnea-hypopnea index (AHI ≥5) was seen in 14 (63.6%) children, periodic limb movement syndrome in 20 (91%) and poor sleep efficacy in 9 (40.9%) children. Ambulatory blood pressure was abnormal in 15 (68.2%) children with CKD. Of them, 4 (18.2%) had ambulatory hypertension, 9 (40.9%) had severe ambulatory hypertension and 2 (9.1%) had masked hypertension. A statistically significant correlation of sleep efficiency with nighttime DBP SD score/Z score (SDS/Z) (r = -0.47; P = 0.02); estimated glomerular filtration rate with SBP loads (r = -0.61; P < 0.012); DBP loads (r = -0.63; P < ) and BMI with SBP load (r = 0.46; P = 0.012) was found.
    Conclusion: Our preliminary findings suggest that ambulatory blood pressure abnormalities, OSA, periodic limb movement syndrome and poor sleep efficiency are highly prevalent in children with CKD stages 3-5.

    Abstract The present study was conducted to assess the extent of induced genetic variability and to determine the inheritance pattern of various yield contributing phenotypic traits in M2 and M3 generations of urdbean following mutagenesis with single and combination treatments of gamma rays and ethyl methanesulphonate (EMS). The mean number of seeds per pod and 100-seed weight increased in all the mutagenic treatments in both the varieties with few exceptions in M2 generation. Mean pod length although increased considerably, however it did not differ significantly in most of the mutagenic treatments. In M3 generation, 0.2% EMS and 300 Gy γ rays+0.2% EMS treatments induced maximum increase in mean pod length, seeds per pod and 100-seed weight in both the varieties. Genetic parameters showed manifold increase in most of the mutagenic treatments and varied from trait to trait. Increased genetic variability for economically important traits in the selected mutant lines has successfully contributed in diversifying the accessible genetic base which could be exploited for subsequent improvement of urdbean through phenotypic selection.

    Abstract Present investigation was carried out to explore the possibility of inducing genetic variability for yield and yield contributing traits in well-adapted variety PU-19 of black gram (Vigna mungo (L.) Hepper) following mutagenesis with methyl methane sulfonate (MMS), sodium azide (SA) and hydrazine hydrate (HZ). A considerable increase in mean values for fertile branches per plant, pods per plant and total plant yield was noticed among the mutant lines in M4 and M5 generations. Estimates of genotypic coefficient of variation, heritability and genetic advance for yield and yield components were also recorded to be higher compared to control. MMS followed by SA and HZ showed highest mutagenic potential for improving total plant yield of black gram var. PU-19. Treatment concentration 0.3% was found to be most effective in generating significant increase in total plant yield of black gram var. PU-19. The increased genetic variability for yield and yield components indicates the ample scope of selection for superior mutants in subsequent generations due to preponderance of additive gene action.

    Abstract Purpose: To compare short-term visual outcomes (best corrected visual acuity [BCVA]), visual axis opacification, anterior (ACCC) and posterior (PCCC) continuous curvilinear capsulorhexis size, shape, and extension, and their decentration between manual capsulorhexis and 25-gauge vitrectorhexis in pediatric cataract surgery with intraocular lens (IOL) implantation.
    Methods: Thirty eyes of children aged 3 to 8 years with developmental cataract were randomly selected for ACCC and PCCC by manual capsulorhexis forceps and 25-gauge vitrectomy cutter followed by IOL implantation and limited anterior vitrectomy. The size of the ACCC and PCCC was measured intraoperatively with calibrated capsulorhexis forceps. Patients were followed up for 3 months postoperatively and were evaluated for BCVA and visual axis opacification. Slit-lamp photographs of operated eyes were taken in retroillumination. The size in millimeters and decentration of the ACCC and PCCC from the center of the IOL were measured with the help of the Python imaging library.
    Results: There was no statistically significant difference between BCVA (P > .05), visual axis opacification (P > .05), size of the ACCC (P > .05) and its decentration (P > .05), extension of the rhexis (P > .05), and size of the PCCC (P > .05) and its decentration (P > .05) between the two methods.
    Conclusions: In both groups, BCVA, visual axis opacification, and ACCC and PCCC size, shape, and decentration from the center of the IOL were comparable, making 25-gauge vitrectorhexis a good alternative to manual capsulorhexis. [J Pediatr Ophthalmol Strabismus. 2019;56(5):327-332.].
