    Abstract Scientific estimations and the dynamic monitoring of the development trend of carbon emissions from energy consumption with a long time series can provide the scientific basis for formulating and implementing regional carbon-reduction strategies. Based on DMSP-OLS and NPP-VIIRS night-time light data, a pixel-scale estimation model of energy-consumption carbon emissions in Jiangsu Province from 2000 to 2019 was constructed. The spatiotemporal evolution characteristics and influencing factors were analyzed using the GIS method and a GTWR (geographically and temporally weighted regression) model. The results showed that: (1) The goodness of fit of the image-fusion correction of the two night-time light data sources from 2012 to 2013 was 0.894; the goodness of fit of the carbon-emission estimation model by stages was above 0.99; and the average relative error was 7.71%, which met the requirement for the estimation accuracy. (2) During the study period, the total carbon emissions from energy consumption in Jiangsu Province continued to increase, rising from 94.7618 million tons to 313.3576 million tons, with an annual growth rate of 6.50%; and the growth rate presented an upward trend of “slow-accelerate-decelerate”. Spatially, it showed an unbalanced distribution pattern of “low north and high south”. (3) Per-capita GDP and energy intensity were the core driving factors affecting carbon emissions in Jiangsu Province over the past 20 years. Energy intensity had the greatest driving effect on carbon emissions in southern Jiangsu, while per-capita GDP had the greatest influence in central and northern Jiangsu. Coordinating the relationship between central, north, and south Jiangsu is of great significance for the realization of the sustainable economic and social development of the double carbon goal.

    Abstract Purpose: Preoperative prudent patient selection plays a crucial role in knee osteoarthritis management but faces challenges in appropriate referrals such as total knee arthroplasty (TKA), unicompartmental knee arthroplasty (UKA) and nonoperative intervention. Deep learning (DL) techniques can build prediction models for treatment decision-making. The aim is to develop and evaluate a knee arthroplasty prediction pipeline using three-view X-rays to determine the suitable candidates for TKA, UKA or are not arthroplasty candidates.
    Methods: A study was conducted using three-view (anterior-posterior, lateral and patellar) X-rays and surgical data of patients undergoing TKA, UKA or nonarthroplasty interventions from sites A and B. Data from site A were used to derive and validate models. Data from site B were used as external test set. A DL pipeline combining YOLOv3 and ResNet-18 with confident learning (CL) was developed. Multiview Convolutional Neural Network, EfficientNet-b4, ResNet-101 and the proposed model without CL were also trained and tested. The models were evaluated using metrics such as area under the receiver operating characteristic curve (AUC), accuracy, precision, specificity, sensitivity and F1 score.
    Results: The data set comprised a total of 1779 knees. Of which 1645 knees were from site A as a derivation set and an internal validation cohort. The external validation cohort consisted of 134 knees. The internal validation cohort demonstrated superior performance for the proposed model augmented with CL, achieving an AUC of 0.94 and an accuracy of 85.9%. External validation further confirmed the model's generalisation, with an AUC of 0.93 and an accuracy of 82.1%. Comparative analysis with other neural network models showed the proposed model's superiority.
    Conclusions: The proposed DL pipeline, integrating YOLOv3, ResNet-18 and CL, provides accurate predictions for knee arthroplasty candidates based on three-view X-rays. This prediction model could be useful in performing decision making for the type of arthroplasty procedure in an automated fashion.

    Abstract Rationale: Anti-N-methyl-D-aspartate receptor (NMDAR) is the most common type of autoimmune encephalitis mediated by NMDAR antibodies. The pathological process remains unclear, especially in patients without tumors or infections. Autopsy and biopsy studies have rarely been reported because of the favorable prognosis. Pathological findings generally demonstrate mild-to-moderate inflammation. This case report presents severe anti-NMDAR encephalitis in a 43-year-old man without any identified triggers. The biopsy in this patient showed extensive inflammatory infiltration with evident B cell accumulation, which enriches the pathological study of male anti-NMDAR encephalitis patients without comorbidities.
    Patient concerns: A 43-year-old previously healthy man presented with new-onset seizures with recurrent jerks. The initial autoimmune antibody test with serum and cerebrospinal fluid yielded negative results. After ineffective treatment for viral encephalitis, based on the imaging results indicating the possibility of diffuse glioma, the patient underwent a brain biopsy in the right frontal lobe to rule out malignancy.
    Diagnosis: The immunohistochemical study showed extensive inflammatory cell infiltration, consistent with the pathological changes in encephalitis. Cerebrospinal fluid and serum samples were then retested and tested positive for IgG antibodies against NMDAR. Therefore, the patient was diagnosed with anti-NMDAR encephalitis.
    Interventions: The patient was administered intravenous immunoglobulin (0.4 g/kg/d for 5 days), intravenous methylprednisolone (1 g/d for 5 days, 500 mg/d for 5 days, subsequently reduced to oral administration), and intravenous cyclophosphamide cycles.
    Outcomes: The patient developed refractory epilepsy 6 weeks later and required mechanical ventilation. Despite brief clinical improvement after extensive immunotherapy, the patient died from bradycardia and circulation.
    Lessons: Anti-NMDAR encephalitis cannot be ruled out even if the initial autoantibody test result is negative. For progressive encephalitis of unknown etiology, it is necessary to recheck cerebrospinal fluid for anti-NMDAR antibodies.
