    Abstract Background: Many patients with allergic rhinitis (AR) have moderate-to-severe persistent AR. Meda Pharma's AzeFlu (MP-AzeFlu
    Methods: This prospective observational study of 214 adults and adolescents in Austria with moderate-to-severe persistent AR assessed the effectiveness of MP-AzeFlu (one spray/nostril twice daily; daily doses: azelastine hydrochloride 548 μg; and fluticasone propionate 200 μg) for AR control in clinical practice using the visual analog scale. Symptom severity was reported on days 0, 1, 3, 7, 14, 21, 28, 35, and 42. Patient demographics, AR phenotype, allergen sensitization, symptomatology, AR treatments in the previous year, and the reason for the MP-AzeFlu prescription were recorded.
    Results: MP-AzeFlu treatment was associated with a rapid and statistically significant reduction in the visual analog scale score from baseline to each timepoint measured, including day 1 (all p < 0.0001). Mean (standard deviation) visual analog scale score was 53.5 mm (26.3) at baseline, 25.3 mm (21.0) on day 28, and 19.6 mm (17.4) on day 42, a mean overall reduction from baseline of 41.4 (23.9) mm for completers. Results were consistent irrespective of patient age, gender, severity, or traditional AR phenotype. Prior to MP-AzeFlu prescription, congestion was considered the most bothersome symptom. The majority of patients reported using at least two AR therapies in the past year, including oral antihistamines, intranasal corticosteroids, and intranasal antihistamines.
    Conclusions: Many patients in Austria live with uncontrolled persistent AR despite treatment. MP-AzeFlu provides effective and rapid control of persistent AR in a real-world Austrian setting.

    Abstract Purpose: To demonstrate how the efficiency of the treatment planning processes of a university radiation oncology department (2,500 new patients/year) could be improved by constructing and implementing a workflow-monitoring application.
    Methods: A web-based application was developed in house, which enhanced the process management tools of the clinic's oncology information system. The application calculates the days left for the next task in the treatment planning process and visualizes the information on a browser-based whiteboard. Workflow monitoring considers tumor types (breast, prostate, lung, etc) and treatment techniques and is backward planned from the planned start of treatment. The effect of introducing this application was analyzed over four phases: (1) baseline data without the workflow-monitoring application, (2) after introducing workflow visualization via a browser-based whiteboard, (3) after upgrading the whiteboard and introducing backend rules, and (4) after updating these rules on the basis of data from the previous phase.
    Results: Implementing the workflow-monitoring application and the introduced measures significantly reduced delays and, consequently, stress and a negative working atmosphere in the treatment planning process. Most notably, the amount of last-minute physics checks (on the day of the treatment start) could be reduced by 50%.
    Conclusion: The study showed what measures can help organize and prioritize the treatment planning workflow. The increased efficiency is believed to improve the quality and reduce the risk of human error.

    Abstract The continuous decline and degradation of tropical rainforests is currently mainly driven by land-use change. Over the long term, climate change will further exacerbate the situation for the remaining forests. To develop sustainable conservation strategies, we need to understand the natural forest-recovery potential after disturbance and the impact of climate on forest recovery. So far, little is known about regeneration of woody species in tropical landscapes, especially in tropical Africa. We investigated the regeneration of woody plant species in six natural and seven anthropogenically disturbed habitat types along a 3.6 km elevational gradient from savanna woodland to afro-alpine vegetation at Mt. Kilimanjaro, Tanzania. We recorded and identified all saplings of woody plants in a total of 65 plots, each measuring 100 m². The total number of recorded sapling individuals amounts to 4,738 saplings from 110 species of 50 families. Along the elevational gradient, sapling numbers in natural plots showed a hump-shaped distribution peaking at mid elevation, where rainfall and overall woody biomass were highest, while species number and diversity of saplings were continuously decreasing with increasing elevation. The sapling layer in the savanna zone had three times as many species as the layer of adult trees and shrubs in the same plots, but the species composition of saplings differed strongly from the one of trees and shrubs, indicating strong spatial or temporal heterogeneity. Active agricultural plots at lower elevations also showed natural regeneration of woody species, even though numbers of sapling individuals and species were much lower than in natural low-elevation plots. Forests formerly disturbed by fire and logging showed either comparable or even increased numbers of sapling individuals and species than natural plots at the same elevation, indicating ongoing succession. Finding regeneration of woody species in all disturbed habitat types, even in agriculturally managed habitats, strongly suggests that natural regeneration is a very powerful tool to recover forest diversity after disturbances. Furthermore, our sapling dataset provides an important baseline for future monitoring of the consequences of climate and land-use change on regeneration.
