    Abstract Thresholding is a prerequisite for many computer vision algorithms. By suppressing the background in an image, one can remove unnecessary information and shift one's focus to the object of inspection. We propose a two-stage histogram-based background suppression technique based on the chromaticity of the image pixels. The method is unsupervised, fully automated, and does not need any training or ground-truth data. The performance of the proposed method was evaluated using a printed circuit assembly (PCA) board dataset and the University of Waterloo skin cancer dataset. Accurately performing background suppression in PCA boards facilitates the inspection of digital images with small objects of interest, such as text or microcontrollers on a PCA board. The segmentation of skin cancer lesions will help doctors to automate skin cancer detection. The results showed a clear and robust background-foreground separation across various sample images under different camera or lighting conditions, which the naked implementation of existing state-of-the-art thresholding methods could not achieve.

    Abstract Purpose of review: Recognition of cardiac arrest and initiation of cardiopulmonary resuscitation (CPR) can be learned and adequately replicated by schoolchildren. Regular instruction of schoolchildren in CPR is therefore a core element to increase low bystander CPR rates. Thereby, schoolchildren CPR training evolved as own scientific field within the last decade. Aim was to describe current evidence in terms of epidemiology, teaching approaches and political aspects.
    Recent findings: Schoolchildren demonstrate a high motivation to be trained in CPR. Teaching approaches that combine theoretical and practical learning sessions guarantee a sustainable learning effect. Schoolchildren can adequately perform chest compressions and mouth-to-mouth ventilation from the age of 12 years. Use of digital media is a highly promising teaching approach. CPR training conducted by teachers from the own school is effective and guarantees continuous development of CPR skills. Integration of schoolchildren CPR training into school curricula is the foundation for a sustainable increase of lay resuscitation rates in the population. Scientific and political promotion of schoolchildren CPR training is needed to sensitize the population and move bystander CPR in the social focus.
    Summary: While bystander CPR rates are low in Europe comprehensive establishment of schoolchildren CPR training may sustainably increase survival after cardiac arrest.

    Abstract Emergency airway management is a rare but essential emergency medical intervention directly impacting morbidity and mortality of emergency patients. The success of airway management depends on various factors such as patient anatomy, environmental aspects and the provider performing the procedure. Therefore, the use of a clearly structured algorithm for anticipating the difficult airway in emergency situations is strongly recommended. Our article explains different ways of securing the airway as part of a structured algorithm as well as pitfalls and helpful tips.

    Abstract Background: Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has spread worldwide. Consequences of infection prevention measures during such contagion events can cause disadvantages especially for patients in out-of-hospital cardiac arrest (OHCA).
    Methods: Retrospective analysis of OHCAs in one county from January-May in 2018, 2019 and 2020, with the first appearance of the SARS-CoV‑2 pandemic in 2020 and a high incidence of the influenza virus in 2018.
    Results: A total of 497 OHCAs were investigated (2018
    Discussion: The lockdown during the first wave of SARS-CoV‑2 pandemic does not seem to have affected the outcome of OHCA patients without coronavirus disease 2019 (COVID-19) in the end.

    Abstract The BrainScaleS-2 (BSS-2) Neuromorphic Computing System currently consists of multiple single-chip setups, which are connected to a compute cluster via Gigabit-Ethernet network technology. This is convenient for small experiments, where the neural networks fit into a single chip. When modeling networks of larger size, neurons have to be connected across chip boundaries. We implement these connections for BSS-2 using the EXTOLL networking technology. This provides high bandwidths and low latencies, as well as high message rates. Here, we describe the targeted pulse-routing implementation and required extensions to the BSS-2 software stack. We as well demonstrate feed-forward pulse-routing on BSS-2 using a scaled-down version without temporal merging.

    Abstract Background: In vitro immunoglobulin E (IgE) tests can be better standardized if based on molecules rather than extracts. However, singleplex screening tests for respiratory or food allergies are still based on extracts only.
    Target: To validate a novel singleplex IgE screening test for respiratory allergies, based on a mix of major allergenic molecules Der p 1, Der p 2, Fel d 1, Can f 1, Can f 2, Can f 3, Can f 5, Bet v 1, Phl p 1, and Art v 1 (Molecular SX01, NOVEOS, HYCOR, USA), and requiring only four microliters (μl) of serum.
    Methods: We examined six subsets of sera from participants of the German Multicenter Allergy Study (MAS) birth cohort enrolling 1314 newborns during 1990: (1) monosensitized (n = 58); (2) polysensitized (n = 24); (3) nonsensitized, with total IgE levels above (n = 24) or (4) below (n = 24) 300 kU/L; (5) sensitized to milk and/or egg but not to airborne allergens (n = 24); and (6) sera of children aged ≤5 years at their earliest IgE monosensitization to airborne allergens (n = 41). Sera were analyzed with the novel molecular SX01 test (NOVEOS) and with three categories of comparators: ImmunoCAP Phadiatop SX01, extracts, and molecules of D. pteronyssinus, cat, dog, grass, and birch. Sensitivity, specificity, positive and negative predictive values were calculated. Quantitative interrelationships were determined using Spearman's rank-order correlation coefficient and Bland-Altmann plots.
    Results: The molecular SX01 test predicted the outcome of IgE tests based on molecules, extracts, or Phadiatop in 188 (96.4%), 171 (87.7%), and 171 (87.7%) of the 195 sera, respectively. Accordingly, sensitivity was 93.5%, 89.0%, and 82.4%, whereas specificity was 100%, 97.6%, and 96.1% when compared with molecular, extract, and Phadiatop tests, respectively. Inconsistent outcomes were largely confined to sera with IgE-Ab levels around the cutoff value of 0.35 kU/L, except for 5/195 (2.5%) sera, containing high levels of IgE to Phl p 5 and/or Alt a 1 only. IgE levels measured by the molecular SX01 test and with IgE tests to molecules, extracts, and Phadiatop were highly correlated (rho 0.90; p < .001), (rho 0.87, p < .001), (rho 0.84, p < .001), respectively. The novel molecular SX01 test detected IgE-Ab in 27/28 (sensitivity 96.4%) of the sera of preschool children at their earliest IgE sensitization to the same molecules.
    Discussion: Our study validates the prototype of a novel category of IgE test, based on molecular mixes. The test's rather good precision and accuracy in early screening IgE sensitization to airborne allergens in German children may be further improved by adding a few other molecules, such as Phl p 5 and Alt a 1.

    Abstract Es wird der Fall eines jungen Patienten mit Kreislaufstillstand nach suizidaler Inkorporation von Natriumnitrat beschrieben. Während der Reanimation sind reversible Ursachen zu suchen und schnell zu therapieren. Intoxikationen sind dabei oft nur schwer detektierbar. Nitroverbindungen führen zur Bildung von Methämoglobin und so zur Hypoxie. Anhand mobiler Laborgeräte kann dies bereits prähospital diagnostiziert werden. Therapeutisch ist zunächst ein symptomatisches Vorgehen mit Oxygenierung und Kreislaufunterstützung sowie ggf. erweiterten Maßnahmen angezeigt. Auch für seltene Intoxikationen sollten spezifische Antidota zumindest zentral vorgehalten werden. Notfallmediziner müssen die Zugriffs- und alternative Behandlungsmöglichkeiten bis zu deren Verfügbarkeit kennen. Im Falle eines Kreislaufstillstands können Systeme zur extrakorporalen Kreislaufunterstützung diesen Zeitraum überbrücken.
