    Abstract Introduction/Objective. As of recently, an increasing number of premenopausal women is being diagnosed with endometrial carcinomas. The objective of our study was to determine if routinely collected clinical and imaging parameters, implying on tumor characteristics, are different in pre- and postmenopausal endometrial carcinoma patients, enabling their appropriate preoperative evaluation. Methods. The study included all patients (n = 209) operated on due to endometrial carcinoma over a period of three years. The diagnosis was based on histopathological findings of exploratory curettage. Medical history was taken for all the patients and they were divided regarding menopausal status. On preoperative ultrasound scan, the endometrial echo pattern was established. The existence of myomas, adnexal masses, free fluid in the abdomen or uterine cavity was noted. Magnetic resonance imaging detected the presence of pelvic metastases and tumor spreading into the uterine cavity, myometrium, cervix, and lymph nodes. Postoperatively, histopathological findings, the tumor stage and grade were established. Results. The majority of women were postmenopausal and secundiparous. Significantly more patients were obese, especially the postmenopausal ones (p = 0.001). Most tumors were endometrioid adenocarcinomas regardless of menopausal status. Irregular/abnormal bleeding (p = 0.037), presence of ascites (p = 0.010), obesity (p = 0.046), and lower parity (p = 0.016) correlated with postmenopausal status. Large exophytic endometrial carcinomas were predominant in younger patients (p = 0.026). Endometrial carcinomas were significantly more often diagnosed in the II FIGO stage in premenopausal patients. There were no other significant differences (endometrial thickness, uterine homogeneity, echogenicity, tumor infiltration and spreading, histopathological type and grade) between pre- and postmenopausal endometrial carcinoma patients.     Abstract Lymphoepithelioma-like carcinoma of the skin (LE-lCS) is a very rare, primary skin neoplasm of uncertain origin. The tumor reveals striking morphological similarity to undifferentiated nasopharyngeal carcinoma (lymphoepithelioma). However, the primary skin tumors have never been associated with Epstein-Barr virus (EBV), in contrast to those at some extracutaneous sites. Since the first description by Swanson et al. in 1988, only 46 cases have been documented over the last 20 years. This case report presents an 89-year-old woman with a 0.5 cm plaque-like, discolored lesion on her left cheek of one-month duration. The patient underwent surgical excision and is disease free one year later. The histopathological, immunohistochemical and EBV in situ hybridization features are described. The clinical and pathological differential diagnosis is discussed. This case supports the contention that LE-lCS is not associated with EBV.

Cutaneous hamartomas containing only neural and/or neurovascular elements are very rare, with very few case reports in the literature.     Abstract Introduction. Adnexal torsion is a rare cause of acute abdominal pain during pregnancy. The clinical and laboratory findings are non-specific. In this paper we present a case of adnexal torsion in the first trimester of pregnancy. Case Outline. On admission, the patient presented signs of acute abdomen. The pain started few hours prior to admission, and was predominantly localized, occasionally irradiating to the central parts of the lower abdomen, accompanied by nausea and vomiting. Ultrasound revealed viable intrauterine pregnancy and right adnexal mass with small amount of free fluid in the Douglas pouch. After short preoperative evaluation, laparotomy and adnexectomy were performed. Surgery and postoperative follow-up were uneventful, and histopathology reported torquated corpus luteum cysts. Conclusion. The diagnosis of adnexal torsion during pregnancy is difficult, and occasionally remains a diagnostic dilemma. Surgery is inevitable, must be prompt, and comprises adnexectomy.

    Abstract Nasal angiomas are rare. We report a case showing osseous metaplasia and discuss pathogenesis hypotheses. A 41-year-old woman presented with a right lateronasal firm, immobile mass, and interfering with glass wearing. The computed tomography scan imaging was suggestive of chondroma while the magnetic resonance imaging showed on T1-weighted images nodule isosignal, on T2-weighted images hypersignal, and intense enhancement after contrast substance injection. The lesion was surgically resected. Histological examination revealed a 0.8 cm angioma with multifocal osseous metaplasia. The diagnosis of nasal angiomas with extensive osseous metaplasia is difficult requiring microscopic examination. Conservative surgery is the treatment of choice even at an early stage due to the limited effectiveness of embolization or drugs on the osseous component.

    Abstract Lupus mastitis represents a subset of breast-limited lupus panniculitis, and occurs very rarely in patients with systemic lupus erythematosus. We report a case of a 33-year-old white woman with a previous history of systemic lupus erythematosus, complaining of an acute tenderness and pain in the right breast associated with localized skin erythema. Ultrasound and mammography reports from a private breast clinic, where she was scheduled for core needle biopsy, aroused suspicion of a breast tumor. Clinically the patient had a painful mass suggestive of an inflammatory lump with an abscess. Her double-stranded DNA antibody level, white blood cell count and erythrocyte sedimentation rate (ESR) were increased; and her renal function was impaired and serology findings negative. All other laboratory tests were within the normal ranges. The patient underwent urgent surgical incision and excisional biopsy of the lesion, and histology revealed lupus mastitis. Lupus mastitis very rarely may appear as a breast lump and therefore presents a great diagnostic challenge for clinicians, occasionally for radiologists and rarely for pathologists.

    Abstract Introduction. Fournier's gangrene (FG) is a fulminant form of infective necrotizing fascitis of the perineal, genital, or perianal regions caused by anorectal or urogenital and perineal trauma, more common in patients with diabetes mellitus and alcohol misuse. Despite contemporary management, mortality is still high and averages 20-30%. Case Outline. A 53-year-old extremely obese diabetic postmenopausal woman with the history of poorly regulated insulin-dependent diabetes mellitus lasting for 20 years, and gangrene followed by autoamputation of minor labia a year ago, was admitted due to severe suprapubic pain and septic fever. Gynaecological examination revealed suprapubic abscess spontaneously perforated few hours prior to admission. Contraincision was performed and purulent green-yellowish discharge appeared. Microbial evaluation of the obtained specimen identified Peptostreptoccocus, Enterococcus and E.coli. Antibiotics were administered according to the sensitivity test and suprapubic tumour formation was irrigated and drained. The procedure lasted seven days and resulted in the healing of the pathological process. Conclusion. FG is still a life-threatening condition; so, diagnosis should be prompt with early surgical intervention, along with antibiotics and good supportive care. Proactive management of the diabetic patient with perineal infections is of extreme importance, in order to prevent the development of FG, the entity associated with very high mortality. According to the literature data available, this is the first reported case of FG relapse.
