    Abstract This paper studies if and to which extent COVID-19 epidemics can be controlled by authorities taking decisions on public health measures on the basis of daily reports of swab test results, active cases and total cases. A suitably simplified process model is derived to support the controllability analysis, highlighting the presence of very significant time delay; the model is validated with data from several outbreaks. The analysis shows that suppression strategies can be effective if strong enough and enacted early on. It also shows how mitigation strategies can fail because of the combination of delay, unstable dynamics, and uncertainty in the feedback loop; approximate conditions based on the theory of limitation of linear control are given for feedback control to be feasible.

    Abstract As the availability of portable echocardiographic equipment is becoming more and more widespread, physicians can add a powerful tool to their bedside examination skills, thus helping them to more effectively face the growing complexity of patients admitted to internal medicine departments or the emergency room. Focused cardiac ultrasound (FoCUS) can be defined as a goal-directed, simplified, qualitative examination performed at the bedside using portable echocardiographic devices. FoCUS is not meant to be a substitute for a standard 2D color Doppler echocardiographic examination. Nevertheless, it can provide rapid and reliable information when limited to basic questions, even when performed by non-cardiologists with brief training. Furthermore, a focused cardiac ultrasound examination maximizes its diagnostic role when integrated with an ultrasonographic assessment of the lung, abdomen and deep veins, in a multisystem approach that is particularly dear to internists. In this article, we will focus on the specific targets of a focused cardiac ultrasound examination, as well as the most common pitfalls that can be encountered in ultrasonographic practice. We will also address the application of FoCUS in the management of two typical scenarios in clinical practice, such as dyspnea and non-post-traumatic hypotension.

    Abstract Aim and objective: Multifocal fractures of the humerus are rare. The aim of our study is to evaluate the effectiveness of surgical treatment and propose a modification to the Maresca-Pascarella classification. A flowchart for surgical treatment is provided.
    Materials and methods: Thirty-one patients with multifocal humeral fractures were treated and evaluated. The Maresca-Pascarella classification was used. All were treated using with either plates and screws, external fixation or intramedullary nailing. Functional outcomes were evaluated using the QuickDASH test, the University of California - Los Angeles (UCLA) shoulder score and the Mayo elbow performance score (MEPS).
    Results: There were 12 Type A, 17 Type B, 1 Type C and 1 of combined fractures of the proximal and distal epiphysis. Of the 31 patients, 5 were lost to the follow-up (FU), 1 died of pulmonary embolism (PE) and the remaining 25 had a mean FU of 19.8 (7-35) months. Three patients had radial nerve damage and 1 went to a non-union that required further surgical intervention. The mean QuickDASH score was 15.7, the average UCLA shoulder score was 26.3 and the mean MEPS elbow score resulted to be 83.0.
    Conclusion: Although multifocal fractures are severe injuries, patients are able to recover good functionality if treated judiciously.
    Clinical significance: We proposed a standardised surgical approach based on the fracture characteristics, site and a modified Maresca-Pascarella classification.

    Abstract Background: Short peripheral catheters (SPCs) are used to provide intravenous therapies in hospitalized patients. Recently, the category of SPC has become more complex, with the introduction in clinical practice of "integrated" SPCs (ISPCs), renewed regarding the material (polyurethane rather than polytetrafluoroethylene) and design (large wing; pre-assembled extension; preassembled needle-free connector (NFC)).
    Methods: This systematic review and meta-analysis aimed to analyze randomized controlled trials (RCTs) and quasi-randomized studies in hospitalized patients, analyzing the risk of overall catheter failure as well as the risk of each type of complication (occlusion, infiltration, thrombophlebitis, and dislodgement) for ISPCs compared to non-integrated SPCs. These systematic review and meta-analysis were registered on PROSPERO (CRD42022322970).
    Data sources: We searched PUBMED
    Results: included studies: The research identified 1260 articles. After the abstract review, 13 studies were included for full manuscript review and, after that, six papers (4727 patients) were included in the meta-analysis.
    Description of the effect: We found a significantly reduced risk of catheter failure (pooling all complications) for ISPCs compared to SPCs (
    Conclusions: ISPCs significantly reduce the risks of catheter failure (overall complications) and occlusion. More RCTs are needed to understand if the preassembled ISPC is better than the composted closed system (non-integrated SPC + extension line + NFC).

    Mit der zunehmenden breiten Verfügbarkeit tragbarer echokardiografischer Geräte können Ärzte eine leistungsstarke Methode hinzufügen, um ihre Fähigkeiten in der bettseitigen Untersuchung zu erweitern. So können sie der wachsenden Komplexität der Patienten, die in den Abteilungen für Innere Medizin oder in der Notaufnahme aufgenommen werden, wirksamer begegnen. Fokussierter kardialer Ultraschall (FoCUS) kann als zielgerichtete, vereinfachte, qualitative Untersuchung definiert werden, die am Krankenbett mittels tragbarer Echokardiografie-Geräte durchgeführt wird. FoCUS ist kein Ersatz für eine standardmäßige echokardiografische 2D-Farbdoppler-Untersuchung. Dennoch kann er bei Beschränkung auf grundlegende Fragestellungen schnelle und zuverlässige Informationen liefern, selbst wenn kurz ausgebildete Nicht-Kardiologen ihn durchführen. Darüber hinaus wird der diagnostische Wert des fokussierten kardialen Ultraschalls maximiert, wenn dieser in eine sonografische Beurteilung von Lunge, Abdomen und tiefen Venen integriert wird, und zwar in einem Multisystemansatz, der Internisten besonders am Herzen liegt. In diesem Artikel konzentrieren wir uns auf die spezifischen Ziele einer fokussierten kardialen Ultraschalluntersuchung sowie auf die häufigsten Fallstricke, die in der Ultraschallpraxis auftreten können. Wir werden uns auch mit der Anwendung von FoCUS bei der Behandlung von 2 typischen Szenarien in der klinischen Praxis befassen – Dyspnoe und nichtposttraumatische Hypotonie.
