    Abstract Objective:Colorectal cancer is the most common type of cancer in the gastrointestinal tract. Preoperative staging is important for applying appropriate treatment modalities. The role of endorectal ultrasonography (ERUS), magnetic resonance imaging (MRI) and positron emission-computed tomography (PET-CT) in rectal cancer patients was evaluated.Method:In this study, between October 2010-April 2012, 30 patients who were diagnosed as rectal cancer clinically and histopathologically were evaluated with ERUS, MRI and PET-CT preoperatively and results were compared with histopathologic findings.Results:Between October 2010-April 2012, 30 patients who were diagnosed as rectal cancer with biopsy in Bezmialem University Medical Faculty Hospital General Surgery Department were included in this study [20 male (66.6%), 10 female (33.3%) and their ages are 38-75 years old, 21 of them received neoadjuvant treatment and 9 of them did not received]. All patients were evaluated with MRI, ERUS and PETCT preoperatively. Rectal cancer patients who are primarily operated from rectal adenocarcinoma were included in this study. For T staging, preoperative MRI, ERUS and PET-CT staged 9 (31%), 12 (41%) and 12 (40%) of 30 patients accurately, respectively. For N staging, preoperative MRI, ERUS and PET-CT staged 15 (51%), 16 (55.1%) and 17 (56.6%) of 30 patients accurately, respectively. In comparison to other modalities, PET-CT did not yield a significant difference in staging and did not change operation strategy. PET-CT detected distant metastasis in 3 patients. One of them was liver and two of them were lung metastasis. Biopsies from mass predicted as lung metastasis did not result as metastasis. PET-CT has high rates of false positivity to detect distant metastasis. In statistical analysis, significant p-values for evaluation could not be obtained.Conclusion:Efficacy of routine use of PET-CT on staging, evaluation of T, N and extramesorectal spread could not be shown.

    Abstract Pilonidal sinus is a very common inflammatory disease of the gluteal region. The ideal method of pilonidal sinus treatment should have a low recurrence rate with minimum excision. Moreover, the treatment method should have a short hospitalization time, should let the patient return to his normal life rapidly, should cause minimum loss of labour and should result a small scar only. In the presented review, modalities in pilonidal sinus treatment in the light of current information in the literature are evaluated.

    Abstract Objective: The imbalance in oxidant/antioxidant status plays a pivotal role in diabetes mellitus (DM). Selenium is a integral component of the antioxidant enzyme glutathione peroxidase. Se treatment induces angiogenesis and improves endothelial function through increased expression of vascular endothelial growth factor (VEGF). The aim of this study is to investigate the effect of selenium on oxidative stress, VEGF, and endothelin 1 (ET1) in a DM rat model. Materials and Methods: We performed an experimental animal study with 64 adult male Wistar-Albino rats. Rats were divided into the following groups (n=8): control (C)7, C21, C+sodium selenite (Se)7, and C+Se21 (control rats), and DM7, DM21, DM+Se7, and DM+Se21 (diabetic rats). Diabetes was induced by 2-deoxy-2-(3-methyl-3-nitrosoureido)- D-glucopyranose [streptozotocin (STZ)]. Three weeks after STZ, DM+Se7 rats received intraperitoneal (i.p.) injections of 0.4 mg/kg Se for 7 days. The DM+Se21 rats received these injections for 21 days. The same dose/duration of Se was administered to the C+Se7 and C+Se21 groups. The remaining rats (C7, C21, DM7, DM21) received physiologic saline injections for 7 or 21 days. Ferric reducing antioxidant power (FRAP), malondialdehyde (MDA), advanced oxidation protein products (AOPP), and endothelial function markers (VEGF and ET1) in plasma samples were measured. Results: Diabetic rats (DM7 and DM21) had significantly increased plasma FRAP (P=0.002, P=0.001), AOPP (P=0.024, P=0.01), MDA (P=0.004, P=0.001), and ET1 (P=0.028, P=0.003) levels compared with C7 and C21 control rats. VEGF (P=0.02, P=0.01) significantly decreased in DM7 and DM21 diabetic rats compared with their controls (C7, C21). Se administration reversed the increased MDA and decreased VEGF levels, and lowered plasma glucose levels in the DM+Se7 and DM+Se21 diabetic groups compared with diabetic rats (DM7, DM21).     Abstract Introduction: Cancer is one of the most important healthcare problems in the world. Boric acid has cytotoxic activity, but there is insufficient data on its effectiveness for cancer development. In this study, we investigated the effect of boric acid on cancer development was investigated in the mouse model for Ehrlich tumor.Methods: In total, 21 female BALB/c mice (mean weight, 27.5 g; mean age, 4 months) were divided into three equal groups. In each group, 1000 Ehrlich tumor cells were implanted intraperitoneally. The first group was intraperitoneally administered 1 mg/day NaH3BO3 solution before Ehrlich tumor implantation. The second group was intraperitoneally administered 1 mg/day NaH3BO3 solution simultaneously with Ehrlich tumor implantation. The third group was intraperitoneally administered 1 ml/day 0.9% NaCl simultaneously with Ehrlich tumor implantation. All administrations continued until the subjects died.Results: None of the rats were sacrificed, and the weight increase time was considered as the starting point of the tumor. There was no significant difference between all groups with respect to tumor initiation times and survival times. Histopathological examinations revealed no significant differences between tumor foci and tumor diameters.Conclusion: The intraperitoneal administration of boric acid in the Ehrlich tumor model showed no effect on cancer development.

    Abstract Introduction: Incisional hernias are abnormal peritoneal outward pouch-like protrusions that develop due to defects that arise as a result of the disruption of the fascia's continuity after abdominal surgery. Presentation of case: A 77-year-old female patient presented to the emergency department of our hospital with complaints of abdominal swelling, abdominal pain, nausea and vomiting. The patient was recommended for surgery. It was decided that the primary fascia closure and onlay patch was the most appropriate approach. Discussion: When the defect in the abdominal wall grows, the functionality of the related abdominal wall is disrupted thereby eliminating the dynamic structure of the abdominal wall. Incisional hernias lead to a significant number of job losses and morbidity and negatively affect quality of life. Moreover, the formations in the hernia pouch might lead to higher risk of strangulation and dysfunction. Conclusion: Subcutaneous herniation of the left lobe of the liver passing through the abdominal wall is a very rare condition.

    Abstract Boerhaave syndrome describes a transmural oesophageal rupture that develops following a spontaneous, sudden intraluminal pressure increase (i.e. vomiting, cough). It has a high rate of mortality and morbidity because of its proximity to the mediastinum and pleura. Perforation localisation and treatment initiation time affect the morbidity and mortality. In this article, we aim to present our successful laparoscopic-endoscopic cooperative surgery in a 59-year-old female who was referred to our clinic with a diagnosis of spontaneous lower oesophageal perforation. Laparoscopy and a simultaneous oesophageal stent application may be assumed as an effective alternative to conventional surgical approaches in cases of spontaneous lower oesophageal perforation.

    Objective: Rare disease.
    Background: Phytobezoars are a common type of bezoar known to accumulate in the gastrointestinal system.
    Case report: A 30-year-old mentally retarded woman was operated on due to small bowel obstruction. A piece of tree root was extracted from the ileum. The patient was discharged from the hospital on postoperative day 7.
    Conclusions: Due to difficulties in determining the nature of the bezoar preoperatively, small bowel obstruction due to a phytobezoar is not a common diagnosis. For bezoars, prevention is considered preferable to treatment.

    Abstract Behçet's disease (BD) is a chronic inflammatory disease. Increased productions of cytokines including interleukin (IL)-1β and IL-18 are documented, and IL-1α and β gene polymorphisms are associated with susceptibility to the disease. IL-33 is a recently discovered member of IL-1 cytokine family. The aim of the study was to detect serum IL-33 level and IL-33 gene polymorphisms in a cohort of BD. Unrelated 117 patients with BD and 149 healthy controls (HC) were enrolled. Serum IL-33 levels were analyzed by enzyme-linked immunosorbent assay method. DNA samples were harvested using an appropriate commercial DNA isolation kit. Four single nucleotide polymorphisms of IL-33 gene (rs7044343, rs1157505, rs11792633 and rs1929992) were genotyped using the appropriate commercial primer/probe sets on real-time PCR. Serum IL-33 level was not significantly different in the BD and HC groups (p > 0.05). However, its level was lower in the active BD patients compared to the inactive ones and HC group (p = 0.044 and p = 0.037, respectively). There was no significant difference in terms of the genotypic and allelic distributions of rs1157505 and rs1929992 polymorphisms (p > 0.05 for all). However, the TT variants of rs7044343 and rs11792633 polymorphisms were very rare, and the T allele frequencies of these polymorphisms were lower, in the BD group compared to the HC group (p < 0.0001 for all). The rs7044343 and rs11792633 variants of IL-33 gene are associated with the decreased risk of BD in our cohort. Therefore, it may be concluded that IL-33 acts a protective role on the pathogenesis of BD.
