    Abstract Chylous ascites is the exudation from lymphatic content to the peritoneum and is a rare situation that mostly occurs following medical causes like neoplasms or cirrhosis. However, trauma to the lymphatic system due to compression by masses or altered anatomy can be a trigger too. We describe a rare combination of a primary small bowel volvulus in a young healthy adult causing chylous ascites. Obstruction caused by a primary small bowell volvulus can re-arrange the lymphatics anatomy increasing their flow pressure which can lead to rupture and leak. This is an emergent scenario that needs to be addressed quickly because of bowel ischaemia. CT scan is the gold standard to expedite diagnosis and go to surgical treatment. Although it can be an impactful finding, treatment of the cause behind chylous ascites results in complete resolution without any bowel resection.

    Abstract Introdução: A amamentação do bebé de mães com COVID-19 pode associar-se a riscos de transmissão do vírus através do leite materno, justificando-se a adoção de práticas seguras. Objetivos: Identificar as recomendações da OMS sobre amamentação e Covid-19. Métodos: Revisão integrativa da literatura realizada em março de 2020 no sítio web da OMS, utilizando-se os descritores “breastfeeding”, “pregnancy”, “childbirth”, “health knowledge, attitudes, practice” e “COVID-19”. Resultados: Existe evidência em sete documentos da OMS que em mulheres com COVID-19 a amamentação deve ser aconselhada, uma vez que o vírus não foi encontrado em amostras do líquido amniótico ou do leite materno, contudo é primordial apoiar a mãe a amamentar com segurança e com medidas de higiene respiratória, lavar as mãos antes e depois de tocar o bebé e higiene das superfícies com que contacta. Conclusões: O conhecimento científico disponível sobre COVID-19 e amamentação encontra-se em constante evolução, devendo os profissionais de saúde apoiar a amamentação e gerir as dificuldades, tomando todas as precauções no sentido de prevenir e controlar o risco de infeção

    Introduction: Breastfeeding the baby of mothers with COVID-19 may be associated with risks of virus transmission through breastmilk, justifying the adoption of safe practices. Objectives: To identify WHO recommendations on breastfeeding and Covid-19. Methods: Integrative literature review conducted in March 2020 on the WHO website using the descriptors “breast feeding”, “pregnancy”, “childbirth”, “health knowledge, attitudes, practice” and “COVID-19”. Results: There is evidence in seven WHO documents that in women with COVID 19 breastfeeding should be advised, since the virus was not found in samples of amniotic fluid or breast milk, however it is essential to support the mother to breastfeed with safety and with respiratory hygiene measures, wash the hands before and after touching the baby and hygiene of surfaces what she contacts. Conclusions: The scientific knowledge available on COVID-19 and breastfeeding is in constant evolution, and the healthprofessionals should support breastfeeding and manage difficulties, taking all precautions in order to prevent and control the risk of infection.

    Introducción: Amamantar al bebé de madres con COVID-19 puede estar asociado con riesgos de transmisión de vírus a través de la leche materna, justificando la adopcion de prácticas seguras. Objetivos: Identificar las recomendaciones de la OMS sobre lactancia materna y Covid-19. Métodos: Revisión integral de la literatura realizada en marzo de 2020 en el sitio web de la OMS, utilizando los descriptores “breastfeeding”, “pregnancy”, “childbirth”, “health knowledge, attitudes, practice” e “COVID-19”. Resultados: Hay evidencia en siete documentos de la OMS que en mujeres con COVID 19 se debe recomendar la lactancia materna, ya que el virus no se encontró en muestras de líquido amniótico o leche materna, sin embargo, es esencial ayudar a la madre a amamentar con seguridade y con medidas de higiene respiratoria, lavar las manos antes y después de tocar al bebé y higiene de las superfícies con la cual contacta. Conclusiones: El conocimiento científico disponible sobre COVID-19 y la lactancia materna esta en evolución constante, y los profesionales de la salud deben apoyar la lactancia materna y manejar las dificultades, tomando todas las precauciones para prevenir y controlar el riesgo de infección.

    Abstract Background: Out-of-pocket expenditure (OOP) are key costs (medical and non-medical) that many individuals incur to receive health services. They have been identified as a key access barrier for vulnerable populations, in particular for populations affected by neglected diseases with a chronic progression, such as Chagas disease. It is important to understand the costs of accessing healthcare services that are borne by patients with T. cruzi infection.
    Methodology: We prepared a structured survey for patients with T. cruzi infection/Chagas disease who were all treated by the healthcare system in endemic municipalities in Colombia. The results were analyzed according to three categories: 1. The socioeconomic profiling of the patients; 2. The costs of accommodation, food and transportation, in addition to the time spent commuting; and 3. the income losses (money that was not earned due to absence from work) related to treatment at the local primary care hospital or at the high-complexity reference hospital.
    Main findings: Ninety-one patients answered the survey voluntarily. The data revealed that, when treated at the specialized reference hospital, patients spent 5.5 times more on food and accommodation, transportation costs were five times higher, and the loss of earnings was three times higher than when they were treated at the local primary care hospital. Moreover, the amount of time spent on transportation was 4 times higher at the reference hospital.
    Conclusions: Providing comprehensive healthcare services for Chagas management at local primary healthcare hospitals would allow the most vulnerable patients to save on expenses related to medical and non-medical costs, in turn leading to higher adhesion to treatment thus benefiting the health system as a whole. These findings are in alignment with the WHO's World Health Assembly 2010 Resolution on the importance of treating Chagas at local primary care hospitals, thereby saving patients time and money, allowing for timely care, and promoting access to healthcare.

    Abstract The recent expansion of multidrug-resistant (MDR) pathogens poses significant challenges in treating healthcare-associated infections. Although antibacterial resistance occurs by numerous mechanisms, active efflux of the drugs is a critical concern. A single species of efflux pump can produce a simultaneous resistance to several drugs. One of the best-studied efflux pumps is the TtgABC: a tripartite resistance-nodulation-division (RND) efflux pump implicated in the intrinsic antibiotic resistance in Pseudomonas putida DOT-T1E. The expression of the TtgABC gene is down-regulated by the HTH-type transcriptional repressor TtgR. In this context, by employing quantum chemistry methods based on the Density Functional Theory (DFT) within the Molecular Fragmentation with Conjugate Caps (MFCC) approach, we investigate the coupling profiles of the transcriptional regulator TtgR in complex with quercetin (QUE), a natural polyphenolic flavonoid, tetracycline (TAC), and chloramphenicol (CLM), two broad-spectrum antimicrobial agents. Our quantum biochemical computational results show the: [i] convergence radius, [ii] total binding energy, [iii] relevance (energetically) of the ligands regions, and [iv] most relevant amino acids residues of the TtgR-QUE/TAC/CLM complexes, pointing out distinctions and similarities among them. These findings improve the understanding of the binding mechanism of effectors and facilitate the development of new chemicals targeting TtgR, helping in the battle against the rise of resistance to antimicrobial drugs. These advances are crucial in the ongoing fight against rising antimicrobial drug resistance, providing hope for a future where healthcare-associated infections can be more beneficially treated.

    Abstract Exploring the effects of meteo-oceanographic (MO) events on ships' maneuverability and safety has great potential, since most maritime accidents occur in confined waters, where the speed of ships is low, and the forces of wind and current on ships have particular importance. Therefore, we put forward a methodology that will be used to qualify and classify the risks caused by MO factors to how ships maneuver, dock or undock in a port. The objective is to generate important information for managing risk. The methodology is validated and illustrated step-by-step by applying it in Suape, one of the most important ports in Brazil, where the docking of larger tankers (e.g., Suezmax) was not allowed until recently when dredging was done to fit the specifications of such ships, thereby expanding the port's operations. MO data on Suape were collected and recorded from September 2016 to November 2017 and used for the application. Based on expert opinion and discussion with a Suape pilot, 36 accidental scenarios (ASs) were identified and categorized using preliminary hazard analysis. From these, the seven most severe ASs were selected so as to assess in more detail the frequency and consequences of accidents on human health, the environment, and property, for which the MO statistics for the likelihood of an accident and/or dispersal of an oil spill were used. The results show that the methodology is viable to assess risks caused by bad weather and to communicate these to pilots and competent authorities, thus improving the safety of operations.
