L’obiettivo è una prima ricostruzione dell’inserimento di alcuni componenti della comunità giudaica peloritana nel lucroso traffico di questa preziosa “merce umana” e dell’atteggiamento dell’aljama nei confronti di importanti itproblematiche come la redenzione degli ebrei ridotti in schiavitù.

    Abstract Fra i centri più popolosi del Valdemone, in Sicilia, Castroreale ha ospitato nel XV secolo una notevole comunità ebraica costituita, in particolar modo, da medici, artigiani e mercanti di tessuti, con strette relazioni commerciali con Messina e il suo patriziato urbano. Nello stesso centro si registra la presenza, da un certo momento in poi, di un significativo numero di neofiti, oggetto di persecuzioni inquisitoriali ancora nei primi decenni del XVI secolo. Judayca terre Castri Regalis: The Jewish presence in a town of the Peloritani (Sicily) in the 15th century Castroreale is a Sicilian centre in Valdemone. It was one of the most populated communities of this large area. In the 15th century, it hosted a Jewish community formed, in particular, by physicians, artisans and merchants of cloth. All were in commercial relations with Messina and its urban patriciate. Castroreale was also home to a significant number of neophytes, who were persecuted by the Inquisition in the first decades of the 16th century.

    Abstract Evidences of Jewish settlements in Sicily date back to the late antique period and this varied and industrious minority has been present, without interruption, until the last decade of the 15th century. The Sicilian Jews lived closely linked to the majority around them, they settled in the three “valli” in which the island was parcelled out and especially in the main coastal cities. Many communities, indeed, grouped around big urban centres and only few of them were situated in suburban areas, even if they were not completely isolated, and many inland communities settled along the main communications lines. The Sicilian Jews devoted themselves to diversified professions; they were physicians, silk weavers, blacksmiths, small artisans, shopkeepers and merchants of several items: cloths, silk, spices, metals and metallurgic products, sugar, slaves and so on.

    Abstract Critical care medicine has been a field for Bayesian networks (BNs) application for investigating relationships among failing organs. Criticisms have been raised on using mortality as the only outcome to determine the treatment efficacy. We aimed to develop a dynamic BN model for detecting interrelationships among failing organs and their progression, not predefining outcomes and omitting hierarchization of organ interactions. We collected data from 850 critically ill patients from the national database used in many intensive care units. We considered as nodes the organ failure assessed by a score as recorded daily. We tested several possible DBNs and used the best bootstrapping results for calculating the strength of arcs and directions. The network structure was learned using a hill climbing method. The parameters of the local distributions were fitted with a maximum of the likelihood algorithm. The network that best satisfied the accuracy requirements included 15 nodes, corresponding to 5 variables measured at three times: ICU admission, second and seventh day of ICU stay. From our findings some organ associations had probabilities higher than 50% to arise at ICU admittance or in the following days persisting over time. Our study provided a network model predicting organ failure associations and their evolution over time. This approach has the potential advantage of detecting and comparing the effects of treatments on organ function.

    Abstract Purpose: After endovascular aneurysm repair (EVAR), an increased [ 18 F]FDG uptake may be observed at PET/CT, being common to both vascular graft/endograft infection (VGEI) and sterile post-surgical inflammation. Increased non-specific metabolic activity, due to foreign body reaction, can persist for several years after surgery, thus complicating the interpretation of PET/CT studies. In this paper, we aimed to assess [ 18 F]FDG distribution at different time-points after the implant of abdominal Endurant ® endografts in patients without suspicion of infection. Methods: We retrospectively evaluated [ 18 F]FDG/CT in 16 oncological patients who underwent abdominal aortic aneurysm exclusion with Endurant ® grafts. Patients had no clinical suspicion of infection and were followed up for at least 24 months after scan. [ 18 F]FDG PET/CT scans were interpreted using both visual and semi-quantitative analyses. Results: The time between the EVAR procedure and [ 18 F]FDG PET/CT ranged between 1 and 36 months. All grafts showed mild and diffuse [ 18 F]FDG uptake without a focal pattern. Mean values of SUVmax were 2.63 ± 0.48 (95% CI 2.38–2.88); for SUVmean 1.90 ± 0.33 (95% CI 1.72–2.08); for T/B ratios 1.43 ± 0.41 (95% CI 1.21–1.65). SUVmax and SUVmean were not correlated to the time elapsed from the procedure, but we observed a declining trend in T/B ratio over time. Conclusions: Endovascular implant of Endurant ® grafts does not cause a significant inflammatory reaction. The evidence of faint and diffuse [ 18 F]FDG uptake along the graft can reliably exclude an infection, even in early post-procedural phases. Therefore, in patients with a low probability of VGEI, [ 18 F]FDG PET/CT can also be performed immediately after EVAR.

    Abstract Pulmonary carcinoids (PCs) are part of a spectrum of well-differentiated neuroendocrine neoplasms (NENs) and are classified as typical carcinoid (TC) and atypical carcinoid (AC). TC differ from AC not only for its histopathological features but also for its “functional imaging pattern” and prognosis. ACs are more undifferentiated and characterized by higher aggressiveness. Positron emission tomography/computed tomography (PET/CT) with somatostatin analogs (SSA) labeled with Gallium-68 ( 68 Ga-DOTA-TOC, 68 Ga-DOTA-NOC, 68 Ga-DOTA-TATE) has widely replaced conventional imaging with gamma camera using 111 In- or 99m Tc-labelled compounds and represents now the gold standard for diagnosis and management of NENs. In this setting, as already described for gastro-entero-pancreatic NENs, 18 F-Fluorodeoxiglucose ([ 18 F]FDG) in addition to 68 Ga-SSA can play an important role in clinical practice, particularly for ACs that show a more aggressive behavior compared to TCs. The aim of this systematic review is to analyze all original studies collected from the PubMed and Scopus databases regarding PCs in which both 68 Ga-SSA PET/CT and [ 18 F]FDG PET/CT were performed in order to evaluate the clinical impact of each imaging modality. The following keywords were used for the research: “ 18 F, 68 Ga and (bronchial carcinoid or carcinoid lung)”. A total of 57 papers were found, of which 17 were duplicates, 8 were reviews, 10 were case reports, and 1 was an editorial. Of the remaining 21 papers, 12 were ineligible because they did not focus on PC or did not compare 68 Ga-SSA and [ 18 F]FDG. We finally retrieved and analyzed nine papers (245 patients with TCs and 110 patients with ACs), and the results highlight the importance of the combined use of 68 Ga-SSA and [ 18 F]FDG PET/CT for the correct management of these neoplasms.

    Abstract PurposeRetinal angiopathy represents a well-known ocular manifestation of hereditary transthyretin amyloidosis (ATTRv). Until recently, there have been no reports on retinal changes in ATTRwt. In this retrospective observational clinical study, we aimed to determine whether vessel density (VD) indices and the choroidal vascularity index (CVI) could offer insights into retinal and choroidal vascular changes among patients affected by ATTRwt.MethodsEighteen patients with a confirmed diagnosis of ATTRwt underwent structural optical coherence tomography (OCT) and OCT angiography (OCTA). We established a control group consisting of 16 healthy subjects for statistical comparisons. The 3D OCT and OCTA datasets were analyzed to assess retinal and choroidal thickness and VD. For measuring CVI, we obtained measurements for the total choroid area (TCA), luminal area (LA), and stromal area (SA).ResultsThe mean VD exhibited a statistically significant reduction in the superficial capillary plexus (SCP), deep capillary plexus (DCP), and choriocapillaris (CC) among the ATTRwt group in comparison to the control group (p < 0.0001). Notably, ATTRwt patients displayed decreased choroidal thickness (p = 0.08). Additionally, the median CVI was lower in the ATTRwt group than in the control group (p = 0.04).ConclusionThe indices from OCTA and CVI have the potential to serve as non-invasive biomarkers for the quantitative evaluation of retinal and choroidal vascular involvement in patients with ATTRwt.

    Abstract Cystic Fibrosis (CF) registries are an essential resource of epidemiological and clinical data. Although the median age at diagnosis is usually reported in the first months of life, a minority of individuals is diagnosed during adulthood. The aim of this study was to describe demographic, genetic, and clinical characteristics of this subgroup of the Italian CF population by using data from the Italian CF Registry (ICFR). Patients ≥18 years at diagnosis were selected and clinical data at diagnosis were analyzed from the 2012–2018 ICFR data (Cohort A). Subjects with diagnosis ≥18 years were selected from 2018 ICFR dataset (Cohort B) to describe their clinical status. In 2012–18 the incidence of late diagnosis was 18.2%, whereas, in 2018, the prevalence of patients diagnosed ≥18 years was 12.54%. The median age of late diagnosis was 36.2 years, ranging from 19.0 to 68.3. The male patients were diagnosed because of infertility in the 45.9% of cases. Median sweat chloride value (SCL) was 69 mmol/L (range 9–150). F508del mutation accounted for 28.3% of alleles. A wide variability in respiratory function was present with a median percent predicted Forced Expiratory Volume in the first second (ppFEV 1 ) of 90.8% (range 20–147%). Low prevalence of pancreatic insufficiency (25%) and of Pseudomonas aeruginosa ( Pa ) infection (17%) suggest a mild CF phenotype in the majority of patients. The assessment of the clinical status in the 2018 dataset and the comparison between genders showed a greater nutritional and respiratory impairment in females. Further studies are needed to clarify the importance of a true diagnostic delay or of late onset of CF symptoms.
