    Abstract The issue of antipsychotic (dis)continuation has been a long-standing clinical dilemma. While the routine usage of antipsychotic is associated with side effects and stigma, short-term evidence suggest that the risk of relapse is heightened following antipsychotics withdrawal. Clinical guidelines therefore propose a one to two years duration of maintenance treatment upon remission in first episode psychosis (FEP), but guidance beyond which remains unclear. Only two controlled studies have addressed the long-term consequences of antipsychotic discontinuation. While Wunderink et al. concluded that dose reduction is associated with a higher rate of recovery, Hui et al. found discontinuation to be associated with better clinical outcomes. Data from Hui et al.'s study further suggests that treatment should be maintained for at least the first three years upon remission in FEP in order reduce the risk of relapse, as well as subsequent poor long-term outcome. It is noted that the two studies not only differ in outcome measures, but also in their strategies of "antipsychotic discontinuation". Considering that discontinuation is a more compelling option to most patients, it may therefore be more clinically relevant. More long-term follow-up discontinuation studies are needed to provide further evidence in the development of treatment guidelines for FEP.

    Abstract Understanding the variations and controls of soil nutrients during different stages of soil evolution can provide guidance for ecosystem nutrient management and regulation. This study investigated the changes in and factors associated with soil nutrients and stoichiometric ratios during different stages of soil development using a well-established soil chronosequence (0.09, 0.146, 0.64, 1.12, 1.81, and 2.30 million years) developed on basalt in southern China. Results showed that available nitrogen (AN) decreased continuously with increasing soil age. Total nitrogen (TN), total phosphorus (TP), available phosphorus (AP), and soil organic carbon (SOC) increased initially during the early stage of soil development, reaching peak values in the 0.146 million-year-old soil profiles. Thereafter, TP and AP fluctuated with increasing soil age, whereas SOC exhibited a decreasing trend with aging soil. The profile-averaged C:N (9.81–12.76), C:P (8.57–23.30), and N:P (0.85–2.00) remained relatively stable across the soil chronosequence (all the ranges < 15), while the profile-averaged AN:AP (29.73–97.36) varied greatly with increasing soil age (the range > 60). Moreover, the decreasing rate of AP (by 64%) and AP:TP (by 71%) was much higher than that of AN (by 46%) and AN:TN (by 26%) when the soil age was greater than 0.64 million years, demonstrating more severe P deficiency and possible greater P limitation in the late stage of soil development. Bayesian structural equation modeling showed that NDVI, average annual evaporation, clay content, and pH considerably influenced the variations in C:N, C:P, N:P, and AN:AP, but the degree of influence differed among the various factors. Our study demonstrates that it is necessary to apply P-fertilizers in older tropical soils (>0.64 million years) developed on basalt to alleviate the increasing P deficiency in the late stage of soil evolution.

    Abstract Helicobacter pylori infection can cause gastritis, gastric or duodenal ulcers, mucosa-associated lymphoid tissue lymphoma, gastric cancer, and extra-gastrointestinal manifestations. Ideal treatment should be guided by antibiotic susceptibility testing. However, this is not feasible in many regions, so the treatment generally relies on clinical experience and regional culture sensitivity profiles. We aimed to integrate the treatment of pediatric H. pylori infection through a systematic literature review. Databases including PubMed, Cochrane Library, EMBASE, and Scholar were searched using terms containing (Helicobacter OR Helicobacter pylori OR H. pylori) AND (child OR pediatric) for all relevant manuscripts and guidelines, published from January 2011 to December 2021. The eradication rate for pediatric H. pylori infection was not satisfactory using triple therapy, sequential therapy, concomitant therapy, bismuth-based quadruple therapy, or adjuvant therapy with probiotics as the first-line therapy. Most therapies could not achieve the recommended eradication rate of >90%, which may be attributed to varying regional antibiotic resistance and possible poor children’s compliance. More studies are required to establish a best practice for pediatric H. pylori infection treatment.

    Abstract Arginine is a semi-essential amino acid that supports protein synthesis to maintain cellular functions. Recent studies suggest that arginine also promotes wound healing, cell division, ammonia metabolism, immune system regulation, and hormone biosynthesis-all of which are critical for tumor growth. These discoveries, coupled with the understanding of cancer cell metabolic reprogramming, have led to renewed interest in arginine deprivation as a new anticancer therapy. Several arginine deprivation strategies have been developed and entered clinical trials. The main principle behind these therapies is that arginine auxotrophic tumors rely on external arginine sources for growth because they carry reduced key arginine-synthesizing enzymes such as argininosuccinate synthase 1 (ASS1) in the intracellular arginine cycle. To obtain anticancer effects, modified arginine-degrading enzymes, such as PEGylated recombinant human arginase 1 (rhArg1-PEG) and arginine deiminase (ADI-PEG 20), have been developed and shown to be safe and effective in clinical trials. They have been tried as a monotherapy or in combination with other existing therapies. This review discusses recent advances in arginine deprivation therapy, including the molecular basis of extracellular arginine degradation leading to tumor cell death, and how this approach could be a valuable addition to the current anticancer arsenal.

    Abstract Vigabatrin (VGB), a potent selective γ-aminobutyric acid transaminase (GABA-T) inhibitor, is an approved non-traditional anti-seizure drug for patients with intractable epilepsy. Nevertheless, its effect on epileptogenesis, and whether this effect is correlated with post-epileptogenic cognitive function remain unclear. Based on lithium-pilocarpine-induced seizure modeling, we evaluated the effect of VGB on epileptogenesis and neuronal damage following status epilepticus in Sprague-Dawley rats. Cognitive evaluations were performed with the aid of inhibitory avoidance testing. We found that VGB could interrupt epileptogenesis by reducing spontaneous recurrent seizures, hippocampal neuronal damage, and chronic mossy fiber sprouting. Nevertheless, VGB did not help with the retention of cognitive performance. Our findings suggest that further research into the role of VGB in epileptogenesis and the treatment of epilepsy in clinical practice is warranted.

    Abstract Hematite (α-Fe₂O₃) nanowhiskers (NWs) synthesized via oxidation of iron-based substrates are a promising photoanode material for photoelectrochemical water splitting. Such synthesized α-Fe₂O₃ NWs have been found to contain ordered axial structures. Herein, we reveal that the known (1[Formula: see text]2)-related ordered structure actually exists in bicrystalline α-Fe₂O₃ NWs instead of single-crystalline α-Fe₂O₃ NWs and that it is associated with another known (3[Formula: see text]0)-related ordered structure. Through a spherical aberration (CS)-corrected high-resolution transmission electron microscopy (HR-TEM) investigation, the microstructural characteristic of the (1[Formula: see text]2)-related ordered structure is verified to be periodic atomic column displacements serving as tensile strain accommodation. The HR-TEM observation are also supported by a monochromated O K-edge EELS analysis, which indicates that α-Fe₂O₃ NWs hosting the (1[Formula: see text]2)-related ordered structure are indeed associated with lattice expansion. In sum, our microstructural study elucidates the root cause of the long-asserted relationship between the (1[Formula: see text]2)-related ordered structure and oxygen vacancy ordering.
