    Abstract Background: The perineal hernia is a condition that occurs as a result of a defect in the pelvic diaphragm. It is classified as anterior or posterior, and as either a primary or secondary hernia. The best management of this condition remains controversial.
    Objectives: To demonstrate the surgical steps of a laparoscopic repair with mesh of a perineal hernia.
    Materials and methods: A video presentation showing the laparoscopic repair of a recurrent perineal hernia.
    Main outcome measures: A 46-year-old woman with a prior history of a primary perineal hernia repair had complaints of a symptomatic vulvar bulge. Pelvic magnetic resonance imaging showed a 5 cm hernia sac at the right anterior pelvic wall containing adipose tissue. A laparoscopic perineal hernia repair was performed by dissection of the space of Retzius, reduction of the hernial sac, closure of the defect and mesh fixation.
    Results: The laparoscopic repair with mesh of a recurrent perineal hernia is demonstrated.
    Conclusion: We showed that the laparoscopic approach can be an effective and reproducible treatment for perineal hernia.

    Abstract Ever since the mid-1960's, locking the phases of modes enabled the generation of laser pulses of duration limited only by the uncertainty principle, opening the field of ultrafast science. In contrast to conventional lasers, mode spacing in random lasers is ill-defined because optical feedback comes from scattering centres at random positions, making it hard to use mode locking in transform limited pulse generation. Here the generation of sub-nanosecond transform-limited pulses from a mode-locked random fibre laser is reported. Rayleigh backscattering from decimetre-long sections of telecom fibre serves as laser feedback, providing narrow spectral selectivity to the Fourier limit. The laser is adjustable in pulse duration (0.34-20 ns), repetition rate (0.714-1.22 MHz) and can be temperature tuned.     Abstract Deforestation in the Amazon rainforest is an alarming problem of global interest. Environmental impacts of this process are countless, but probably the most significant concerns regard the increase in CO 2 emissions and global temperature rise. Currently, the assessment of deforested areas in the Amazon region is a manual task, where people analyse multiple satellite images to quantify the deforestation. We propose a method for automatic deforestation detection based on Deep Learning Neural Networks with dual-attention mechanisms. We employed a siamese architecture to detect deforestation changes between optical images in 2018 and 2019. Experiments were performed to evaluate the relevance and sensitivity of hyperparameter tuning of the loss function and the effects of dual-attention mechanisms (spatial and channel) in predicting deforestation. Experimental results suggest that a proper tuning of the loss function might bring benefits in terms of generalisation. We also show that the spatial attention mechanism is more relevant for deforestation detection than the channel attention mechanism. When both mechanisms are combined, the greatest improvements are found, and we reported an increase of 1.06% in the mean average precision over a baseline.

    Abstract Brillouin dynamic gratings (BDG) can measure the distributed birefringence of polarization-maintaining fibers (PMF), however, its sensing range is limited by both stimulated Brillouin scattering depletion and fiber losses in PMF, which are significantly higher than those in standard single-mode fibers.     Abstract We study, theoretically and empirically, how countries choose intra-bloc tariffs and preferential margins when they form Preferential Trade Agreements (PTAs). Our model indicates that countries should set systematically lower preferential margins when the bloc takes the form of a free trade area (where members set external trade policies independently), relative to a customs union (where members coordinate external tariffs). Moreover, in customs unions (but not necessarily in free trade areas) preferential margins should increase with the supply of partner countries and decrease with the level of preferential imports. These relationships reflect, respec-tively, the internalization of political-economy goals within the bloc and the desire to curb trade diversion. Using a sample that covers most PTAs formed by Latin American countries in the 1990s, when their popularity in the region shot up, we find empirical support for each of those predictions. These findings make clear that the type of PTA matters significantly for the bloc's tariff structure. Our results carry out important implications for the welfare consequences and the social desirability of different types of PTAs.
