    Abstract Dr. Joan Braganza, a world expert in the field of chronic pancreatitis, proposed a new template for its pathogenesis based on the role of free radical pathology, in particular the heightened but unmitigated oxidative detoxification reactions via cytochromes P450. Dr. Braganza has gone on to show how pancreatic damage in cystic fibrosis, acute pancreatitis and pancreatic cancer fit into the scheme, paving the way for new treatment modalities. In this interview, Dr. Braganza shares her life experience as an investigator and provides a perspective for young researchers entering the field of pancreatology.

    Abstract Objective: To describe the design and impact of a systematic, enterprise-wide process for engaging US Department of Veterans Affairs (VA) leadership in prioritizing scarce implementation and evaluation resources.
    Data sources: From 2017 to 2021, the VA Quality Enhancement Research Initiative (QUERI) identified priorities from local, regional, and national leaders through qualitative discussions and a national survey and tracked impacts via reports generated from competitively funded initiatives addressing these priorities.
    Study design: Guided by the Learning Health System framework and QUERI Implementation Roadmap, QUERI engaged stakeholders to nominate and rank-order priorities, peer-reviewed and funded initiatives to scale up and spread evidence-based practices (EBPs) using theory-based implementation strategies, and evaluated the impact of these initiatives using the QUERI Impact Framework.
    Data collection/extraction methods: QUERI collected priority nominations through qualitative discussions and a web-based survey, and live voting was used to rank-order priorities. QUERI-funded teams regularly submitted progress reports describing the key activities, findings, and impacts of the quality improvement (QI) initiatives using a standardized form created in the VA Research Electronic Data Capture (REDCap).
    Principal findings: QUERI launched five QI initiatives to address priorities selected by VA leadership. In partnership with 28 health system leaders, these initiatives are implementing 10 EBPs across 53 sites, supporting 1055 VA employees in delivering evidence-based care. The success of these initiatives led to an expansion of QUERI's process to address 2021 VA leadership priorities: virtual care, health disparities, delayed or suppressed care due to COVID-19, employee burnout, long-term and home care options, and quality and cost of community care.
    Conclusions: QUERI, a unique program embedded in a national integrated health system, deployed a novel approach to inform policy making and enhance the real-world impact of research through prioritization of limited resources, rigorous peer-review, and assessment of impacts on the health system, employees, and Veterans.

    Abstract Pancreatic extracts have found their way into clinical practice to treat painful chronic pancreatitis, despite consensus from analysis of randomized controlled trials (RCT) that there is no clear evidence of benefit [1-3]. Proponents argue that success depends upon the use of non-enteric coated material to ensure delivery of proteases into the duodenum as in two RCT [4, 5], compared to several unsuccessful RCT using enteric coated preparations that deliver the enzymes further downstream [1-3]. It is posited that intraduodenal delivery ‘puts the pancreas to rest’ by dampening the feedback loop that otherwise leads, via high circulating levels of cholecystokinin - pancreozymin (CCK), to pain from pancreatic ductal hypertension. Clinicians are informed that patients with small-duct disease and mild to moderately impaired pancreatic secretory capacity will benefit, whereas those with advanced disease / steatorrhea will not. A proton pump inhibitor is advised, to safeguard the extracts in transit through the stomach. Treatment is advocated for four weeks in the first instance, to be continued for six months in responders, and indefinitely should pain resurface upon attempted withdrawal of treatment [6].

    Abstract Dear Sir, We appreciate the comments of Rajesh et al. published in the present issue of JOP. J Pancreas (Online) [1] in respect of our review on ‘Micronutrient therapy for chronic pancreatitis’ [2] and thank you for the opportunity to respond. The authors address two points in particular. i) Premorbid micronutrient insufficiency (especially of methionine and ascorbic acid) compromises the delivery of methyl and thiol moieties that facilitate exocytosis from acinar cells and leaves the exocrine pancreas vulnerable to other consequences of persistent electrophilic stress. These problems underlie chronic pancreatitis, such that micronutrient therapy affords first-line treatment and gives an opportunity for prophylaxis in individuals/communities at high risk of the disease
