Abstract Abstract Previous experiments performed below 20 GPa suggested that the metal/silicate partition coefficient of phosphorus (P), D P, extrapolated to typical high-pressure and -temperature conditions of the Earth’s core formation gives too high P concentration in the core unless a large amount of silicon was included in metals. Here we examined D P between liquid metal and coexisting molten silicate at 27–61 GPa and 3820–4760 K, corresponding to conditions of core-forming metal segregation from silicate, by measuring recovered samples using a high-resolution imaging technique coupled with secondary ion mass spectrometry. The results demonstrate that the pressure dependence of D P changes from positive to negative above 15 GPa, likely because of an increase in the coordination number of P5+ in silicate melt. With the present new partitioning data, the observed mantle P abundance may indicate ~ 0.2 wt% P in the core, consistent with the cosmo-/geochemical estimates, based on both single-stage and multi-stage core formation models without involving high amounts of silicon in metals.

Abstract Introduction: This study aimed to demonstrate whether benralizumab maintained the safety and effectiveness profiles established in randomized controlled trials among all patients with severe uncontrolled asthma initially prescribed benralizumab in the real-world setting in Japan.
Methods: This was a prospective, observational, multicenter post-marketing study (ClinicalTrial.gov, NCT03588546). The safety and tolerability of benralizumab over 1 year were assessed by the incidence of adverse events (AEs), serious AEs, adverse drug reactions (ADRs), and serious ADRs. Patient background characteristics indicating a more frequent onset of ADRs with benralizumab were explored. The main effectiveness assessment was the change in Asthma Control Questionnaire-5 (ACQ-5) score from baseline. Patients with baseline ACQ-5 scores ≥1.5 were defined as having severe uncontrolled asthma.
Results: In total, 632 patients were evaluated for safety and 274 for effectiveness; 139 patients were included in the severe uncontrolled asthma subgroup. ADRs were reported in 12.7% and serious AEs in 13.0% of patients. Serious infections occurred in 3.8%, serious hypersensitivity in 0.3%, and malignancy in 0.3% of patients. No helminthic infections occurred. In the effectiveness population, benralizumab improved the mean (standard deviation [95% confidence interval]) ACQ-5 score by -1.16 (1.40 [-1.36, -0.96]) from baseline; forced expiratory volume in 1 second by 0.151 (0.440 [0.09, 0.21]) L; and Mini-Asthma Quality of Life questionnaire score by 1.16 (1.29 [0.94, 1.38]) at the last observation. The annual asthma exacerbation rate was 0.42. A greater ACQ-5 score improvement was observed among patients with eosinophilic asthma characteristics.
Conclusion: No new safety concerns were raised, and patients experienced benefits consistent with previous studies of benralizumab, thus supporting the use of benralizumab for the add-on maintenance treatment of patients with eosinophilic severe uncontrolled asthma.

Abstract Vasoactive intestinal peptide-producing tumor of the pancreas (VIPoma) is one of the rarer subtypes of neuroendocrine tumor (NET) of the pancreas. It usually represents intractable diarrhea, weight loss, and electrolyte abnormalities secondary to diarrhea. The most common site of metastasis of VIPoma is the liver. Furthermore, lymph node metastasis (LNM) is rare, and no metachronous LNM with a resectable situation has been reported before. A 60-year-old male patient (height: 181 cm, body weight: 74 kg) with a history of operated pancreatic VIPoma three years ago was referred to our department due to the detection of lymphadenomegaly which was suggestive of lymph node metastasis by routine follow-up computed tomography (CT). Preoperative CT showed a lymph node on the left side of the abdominal aorta and caudal side of the left renal vein with a size of 1 cm. Lymphadenectomy was performed without significant complications and blood loss. This is the first report of metachronous LNM in a patient with operated VIPoma. Although much rarer than solid organ metastasis of VIPoma, LNM in these patients can also be seen synchronously and metachronously. Close follow-up and vigilance are key to preventing recurrence-related morbidity and mortality in these patients.

Abstract Notch signaling is involved in cell-cell communication. It is an evolutionarily ancient mechanism and plays a fundamental role in development. The typical function of Notch signaling is the regulation of cell fate segregation at asymmetric division; however, a role in tumorigenesis has also been suggested. Inactivating mutations of NOTCH1 are present in about 10 % of cases of squamous cell carcinoma of the skin, oral cavity, esophagus, and lung, rendering it one of the most frequently mutated genes in squamous cell carcinoma. Mouse knockout studies have demonstrated that Notch1 is imperative for early development but is dispensable for formation of the squamous epithelium. However, loss of Notch signaling predisposes the epidermis to hyperplasia and increases tumor incidence. This tumor-inducing effect resulting from the loss of Notch signaling is associated with non-cell-autonomous effects that are elicited by subtle alteration of epithelial cell features, generating a wound-like microenvironment in the underlying stroma. We found that Notch1 was expressed specifically in the basal cells of the oral squamous epithelium. In cancer and oral epithelial dysplasia, it was significantly downregulated, suggesting that reduced Notch activity plays a distinct role in oral neoplasia.

Abstract When a tumor to be resected is located close to or extending beyond the intersegmental plane, combined segmentectomy or subsegmentectomy is an option to ensure a sufficient surgical margin. The segmental and subsegmental anatomy of the lungs varies greatly from case to case. Therefore, there is no standardized procedure for combined subsegmentectomy, and surgical planning based on individual anatomy using three-dimensional computed tomography is essential. This video tutorial presents the surgical technique for uniportal video-assisted thoracic surgery of the left S1+2c+S3a combined subsegmentectomy for tumors located at the border of the S1+2c and S3a segments of the left upper lobes, with preoperative three-dimensional simulation and intraoperative navigation images.
