    Abstract This paper considers queer studies in the global geopolitical hotspot of Asia, as well as how we can reimagine queer theories through both the Covid-19 pandemic and the intensified regional and global superpower competition and geopolitical tensions. It argues for a rethinking of queer studies through today's international relations and geopolitical complications in a sociological political economy. The aim is to connect critical studies with analyses of economic and social class structures, an approach that has been substantiated by the current crises, and to present an expanded queer mobility theory with two brief case studies (mini-critiques) of the current socioeconomic conditions facing marginalized people under Covid-19 and the changing geopolitical landscape. In so doing, this paper actively explores what queer studies can do and can be through the current historical turning point of the pandemic and geopolitical rivalry toward potential post-Covid socioeconomic revival and recovery.

    Abstract Purpose of review: The field of urology has been beset by several major trends that have affected the early detection of prostate cancer. These stem primarily from a backlash against overdiagnosis due to prostate specific antigen-based screening efforts and are epitomized by the US Preventive Services Task Force giving prostate specific antigen-based prostate cancer screening a 'D' recommendation. Consequently, the active surveillance strategy for low-risk prostate cancer has become commonplace, leading many to ask how best to follow these patients. More importantly, this public outcry has shifted the focus of early detection from an effort to diagnose any and all prostate cancers to an effort to diagnose only 'high-risk' cancer. Along with a trend for minimally invasive procedures, these forces have challenged the early detection field to more efficiently identify clinically significant prostate cancers at an early stage while limiting the number of biopsies.
    Recent findings: With US Food and Drug Administration approval, prostate cancer antigen 3 has emerged as the first bona-fide urinary biomarker for prostate cancer. Using the same platform, investigators have developed a second urinary test based on TMPRSS2:erg fusion. Recent literature supports the use of these biomarkers as a combined panel that improves risk evaluation in the setting of prostate cancer detection. Early works for applying urinary biomarkers for active surveillance are underway. Other biomarkers in the pipeline will require further prevalidation and validation work.
    Summary: Recent literature would support that urinary biomarkers have a clear role to supplement risk evaluation for men undergoing prostate biopsy and for prognostication.

    Abstract Over the past two decades, there have been significant advances in the treatment of multiple myeloma which has led to an improvement in overall survival.1,2 However, a notable proportion of patients continue to experience early mortality (EM), defined as 2 years from the time of diagnosis. This raises the possibility that improvements in myeloma survival have not extended equally to all groups. Using the latest data drawn from the Surveillance Epidemiology and End Results database of patients in the United States spanning 2000-2019, we study impact of important sociodemographic factors on EM. Through regression modeling, we demonstrate that patients diagnosed from 2000-2005, of older age, male sex, and of certain racial minority status (non-Hispanic Black and Hispanic) have higher odds of EM. Of these factors, minority status contributed to worse 2-year overall survival as well. We evaluate whether income, as a surrogate to access to care, could potentially explain this finding, but find that race has a distinct relationship with EM that is not modified by income. This is further reinforced by subgroup analysis. After characterizing groups vulnerable to EM, we examine reasons for these disparities and potential avenues to address them.

    Abstract Soybeans are the most common source of isoflavones in human food, and have been shown to have anti-inflammatory and anti-oxidant properties. We hypothesize that isoflavones modulate both Th1 and Th2 immune responses by regulating dendritic cell (DC) function. We have addressed this hypothesis with in vitro DC activation and coculture models, and in vivo models of lipopolysaccharide (LPS) and cholera toxin (CT)-induced sensitization to antigen. In vitro, we found significant suppressive effects of isoflavones on DC surface molecule expression and cytokine secretion, and subsequently, the ability of these DCs to induce T cell and natural killer (NK) cell activity. In vivo, we found that isoflavones cause significant suppression of LPS and CT-induced immunity, and that isoflavones may exert effects in their active forms at the site of sensitization, or as conjugates in blood. These results indicate that isoflavones may have a significant role in both Th1 and Th2-mediated sensitization.

    Abstract In an attempt to reduce morbidity, focal targeted therapies and active surveillance have become increasingly popular treatment choices for localized prostate cancer. However, these modalities rely heavily on accurate and reliable tumor localization information provided by a prostate biopsy. Evidence that our contemporary biopsy techniques can do little more than detect some prostate cancers is notably lacking. What is meant by the accuracy and reliability of a prostate biopsy and why they are such important concepts to focal therapy and active surveillance are discussed.
